Skill: Perform and Repair an Episiotomy

Objectives:
1. Types of episiotomy.

1. Preparation Check the medication to be given (lidocaine


0.5% or 1%).
2. Select sterile syringes and needles of appropriate
sizes.
3. Prepare the episiotomy scissors.
4. Inform patient of the procedure.
5. Disinfect top of drug vial.
6. Draw out the medication into the syringe using a
needle, and use forceps to discard the needle into
the sharp bin.
7. Attach a new needle with its cover to the
syringe, and place into a receiver.
8. To protect the fetal head, two fingers are inserted
into the vagina.
9. Insert needle into the perineal tissue at the centre
of the fourchette and should be directed towards
the proposed line of incision.
10. Confirm that needle has not penetrated a blood
vessel by withdrawing the syringe piston.
11. Once again, insert two fingers into the vagina to
protect the fetal head.
12. A single cut is made from the centre of the
fourchette in a mediolateral direction avoiding
damage to the anal sphincter.
13. On completion of the third stage, the vagina,
perineum and rectal areas are assessed for any
missed tears.

15. Clean and drape the perineum and prepare for
repairing of the wound.
16. Local anesthesia is given by inserting needle
under the perineal tissue around the wound.
17. Place the first suture 1 cm above the apex.
18. Bring together the vaginal edges by continuous
sutures placed 1 cm apart.
19. Oppose the divided levator ani muscle by 2-3
interrupted sutures.
20. Close skin by continuous subcuticular sutures or
interrupted sutures.
21. Inspect the vagina to ensure completeness of
repair.
22. Complete the procedure by doing per rectal (PR)
examination.
23. Clean the patient and ensure comfort.
24. Remove gloves, face mask and apron, dispose in
appropriate bin and wash hands.
25. Documentation.
