1 ACCIDNT / INCIDENT REPORTING AND INVESTIGATION

All incident / accident / near miss cases are subject to report to owner in written
within the specified time frame and all such cases shall be investigated by a
team headed by the safety officer. The outcomes (Causatives and corrective
actions) shall be documented as submitted to BALCO with a commitment to
compliance with the proposed time.

ARCI SITE - ACCIDENT/INCIDENT REPORTING SYSTEM

1. Inform the accident/incident to the concern Supervisor/Safety Officer/Site


Incharge.

2. Give the first aid through the authorized person (S.Mahalingam.-Safety


Coordinator).

3. First aid boxes are available at


I) MSDS
II) DG Room

4. For the major accident/incident inform immediately to the Siemens Head


Office and Mr. Jagan Mohan Reddy - Safety Officer, ARCI.

5. Emergency contact numbers


I) Ambulance: 108
II) Hospitals: a) Apollo Hospital : 040 23608856
b) Kaminene Hospital: 040 24022272-76

6. Arrange Ambulance, Stretchers and other facilities.

7. Take him to the Apollo or Kaminene Hospital.

8. In case of emergency reach EMP (Emergency Meeting Point) at Site Office.
