Sepsis Early Goal Directed Therapy (EGDT) Algorithm/Driver Diagram

The document outlines an early goal directed therapy (EGDT) algorithm for treating sepsis. It defines the symptoms of sepsis and criteria for diagnosis. If sepsis is diagnosed based on meeting two or more systemic inflammatory response criteria and a suspected infection, EGDT is started. EGDT involves administering antibiotics within 1 hour, inserting a sepsis catheter within 2 hours, and using intravenous fluids and medications to achieve targets for central venous pressure, mean arterial pressure, and central venous oxygen saturation within 6 hours in order to improve outcomes for patients with sepsis.

(Refer to Sepsis Glossary for definitions of abbreviation such as SIRS, HR, WBC, etc)

Early Identification and Diagnosis Treatment/Management Treatment Goals

Symptoms of Sepsis Include: If a Sepsis diagnosis is determined, EGDT is  Give antibiotics within 1 hr of EGDT
started when the patient’s lactate result is > 4 start time
The patient has 2 or more SIRS criteria: or 1 hr after SBP is < 90 if it remains < 90 after
 HR > 90 1 hr  Sepsis catheter is inserted within 2
 RR > 20 hours of EGDT start time
 Change in LOC EGDT treatment includes:
 Temperature > 100.4 F (38.0 C) or < 96.8 F  Antibiotics  CVP is 8-12 within 6 hr of EGDT start
(36.0 C)  Sepsis catheter time (fluids used to reach target).
 WBC > 12, 000 or < 4,000  Intravenous (IV) Fluids - Once CVP target is met focus on
 IV medication meeting MAP goal
AND - Norepinephrine for low MAP
- Dobutamine to help with oxygen delivery  MAP > 65 within 6 hours of EGDT start
 Known or suspected infection, for example: (low ScvO2) time (fluids and Norepinephrine can be
 Cloudy, foul smelling urine  Blood if Hct < 30 used to reach target)
 Wound with drainage or pus  Repeat lactate test - Once MAP target is met focus on
 Cough with green, yellow, brown sputum meeting ScvO2 goal

If the patient arrives to the ED with these  ScvO2 > 70 within 6 hours of EGDT
symptoms, or develops these symptoms while start time (blood and Dobutamine can
staying in the hospital, then blood cultures and be used to reach target)
lactate levels are drawn and close monitoring of
the blood pressure is done to help confirm the  Draw lactate every 6 hours for 24
diagnosis. hours, goal is that lactate is less than
original value within 12 hours of 1st
Lab results can confirm a Sepsis diagnosis: result
 Positive blood, urine, or sputum culture
 Lactate > 4
 SBP < 90 for more than 1 hr
