I have attached two zip files to this email. The first is called survey.zip. The
second is called report_package.zip. You need to download both of these files to
your computer, double click on each to unzip them, then copy and paste the
unzipped ‘Survey’ folder on to a flash drive. Do not delete the unzipped files off
your laptop.
You then need to bring the flash drive to EACH computer you want the
participants to take the survey at. Plug the flash drive into each computer and
copy and paste the ‘Survey’ folder on to the desktop of each computer.
Once you have copied the folder, you need to do the following actions on EACH
computer:
1. After copying the Survey Folder in the computer, go and open the
“Survey” excel file.
2. You must Enable Macros for the “Survey” excel file:
3. If the system asked to download fonts, please Skip (no need to download
any font).
4. After the file is opened, you will be in the “instruction” sheet and you will
see two buttons:

5. You can now leave that computer as is – it is ready for the participants to
click on “Start Survey” and complete their self-assessment.
6. Once they have completed the survey, go to each computer and click
“Export Data”.
7. Click on the ‘Survey folder on the desktop, and then open the ‘Survey_File’
folder. You will see a file called “Database” followed by numbers. This is
your participant’s data.
8. Plug in your flash drive and copy and paste the “Database” file into your
flash drive. Do this for EACH computer.
9. When you have collected all the data files, go to YOUR computer, plug in
the flash drive and copy ALL the “Database” files from the flash drive into
the “Databases” folder within the “Report Package” folder on your
desktop.
Now take a break! The data files are now all safely on your laptop and you can
view them at your leisure using the steps below:
1. Open the “Reports Package” folder on your desktop. Double click on
the “Summary Reports” Excel file.

4. Click on the “Import Data” button. This may take a number of minutes
to complete. It is importing the data you collected and dividing the
data between Master Teacher and Teacher I-III.
5. Once this is finished, you can click on “Teachers I-III Summary” to
view the Teachers I-III summary with graphs, or you can click on
“Master Teachers Summary” to view the Master Teachers summary
with graphs.
