Nomination and Declaration Form

This document contains forms for nominating beneficiaries to receive an employee's provident fund and pension benefits in the event of their death. Part A allows the employee to nominate one or more individuals to receive the provident fund amount. Part B requires the employee to provide details of family members eligible for widow/children's pension and also nominate an individual to receive the pension if there are no eligible family members. The employer must certify that the employee signed or thumb impressed the forms after reading the entries.
