Object : To report incident of every nature related to health , safety & environment

Person Responsible : All TPPL employees

Other person to be involved : Line managers , HSE focal point

Timing : In case of an incident

Actions to be taken :

1. Written information to be sent to the line manager within 24 hrs of the incident.
2. In case of major / catastrophic incident the matter should be reported within 2
hours.
3. In case of a holiday, verbal communication can do that needs to be followed up by
the written report on the next working day.
4. A detailed incident report is to be submitted on the prescribed format within one
week by the relevant staff.
