I have the honor to seek transfer in my teaching assignment from New Dumangas
National High School to Kusan National High School for the following reasons to wit:

1. That I have served the school for almost 15 years since last June 2003.
2. That I am a bonafide resident of Gayaban Barangay Libertad, Zone 2-A,
Surallah, South Cotabato and I want to be near to my residence considering
its distance from my teaching assignment.
3. That I have two kids and old-aged mother who really need utmost care and
attention.
4. That I am a Junior High School teacher handling English subjects for almost
15 years.
5. That there is a vacant item in English for Junior High School at Kusan NHS.

I am hoping that these aforementioned reasons be given your favorable approval.
