LAST WILL AND TESTAMENT

of
ALBERTO MENDOZA
KNOW ALL MEN BY THESE PRESENTS:
I, ALBERTO MENDOZA, of legal age, single, native of San Narciso, Zambales,

having been born on September 16, 1941, now actually residing at 24 Mindanao
Ave. Ext., Quezon City, being of sound and disposing mind and memory, and not
acting under undue influence, violence, fraud, or intimidation of whatever kind,
do by these presents declare this to be my Last Will and Testament which I have
caused to be written in English, the language which is known to me. And I hereby
declare that:
I. I desire that my remains be buried according to the rites of the

Roman Catholic Church
II. I give and bequeath to my wife, Alexandra Mendoza, the following
property wit:
“A parcel of land (Lot No. 2603 of the Cadastral Survey of San Narciso,
Cadastral Case No. 12, L.R.C. Castral Record No. 377), situated in the
Barrio of San Juan, Municipality of San Narciso, Province of Zambales.
Rounded on the NE by Lot No. 9148; on the SE. by Lot No. 9147; on the
SW. by a road; and on the NW. by Lot no. 6594. Beginning at a point
marked “1” on plan, being N. 7 deg. 47’W., 803.24 m., from N.B.W. No.
21; thence S. 0 deg. 50’E., 6420 m. to point 2; S. 8 deg. 16’W., 10.30m. to
point 3; S. 2 deg. 37’W., 7.67 m. to point 4; N. 73 deg. 32’W., 45.81 m. to
point 5; N. 76 deg. 23’E., 46.99 m. to the point of beginning; containing
an area of FIVE HUNDRED AND FORTY TWO (542) SQUARE METERS,
more or less. All points referred to are indicated on the plan; bearings
true; date of survey, August, 1918-November, 1920.”
III. I give and bequeath to my children, Tito, Vic and Joey, in equal
shares, all my other property, real and personal, whatsoever and
wheresoever located;
IV. I designate ARMANDO CARUSSO the sole executor of this my Last

Will and Testament, and in his default or incapacity to act, my Tito,
and in default or incapacity of the latter to act, one of my other
children in the order of priority in age, if of age;
V. I hereby direct that the executor named herein or his lawful

substitute should serve without bond;
VI. I hereby revoke any and all my other wills, codicils, or testamentary
dispositions heretofore executed, signed, or published, or alleged to
have been executed, signed or published, by me.
IN WITNESS WHEREOF, I have hereunto set my hand this 16 th day of

February, 2016 in Quezon City.

ATTESTATION CLAUSE
We, the undersigned attesting witnesses, whose residences are stated

opposite our respective names, do hereby certify: That the testator, Alberto
Mendoza has published unto us the foregoing will consisting of ___ pages
numbered correlatively in letters on the upper part of each page, as his Last Will
and Testament and has signed the same and every page thereof, on the lef
margin, in our joint presence and we, in turn, at his request have witnessed and
signed the same and every page thereof, on the lef margin, in the presence of the
testator and in the presence of each and all of us.

Witness, Chito Miranda, with Res. Cert. No. 39100 issued at Quezon
City on January 7, 2016.
Witness, Ely Buendia, with Res. Cert. No. 38413 issued at Quezon City
on January 6, 2016.
Witness, Jett Pangan, with Res. Cert. No. 40215 issued at Quezon City
on January 6, 2016.
All known to me to be the same persons who executed the foregoing

instrument, and they acknowledged to me that the same are their free act
and deed.
This Will consists of three (3) pages, including the page on which this
acknowledgment is written, and has been signed on the lef margin of each
and every page thereof by the testator and his witnesses, and sealed with
my notarial seal.
