Health Promotion with Individuals I


Assignment Exploring Health

Purpose

Students will have the opportunity to develop a partial nursing care plan by conducting
a general survey and a partial health history with a well adult in the community.
Students will explore health with that individual and formulate a plan of care based on
that history. The nursing care plan will identify one priority client concern, formulate a
goal (STROMMC) for the concern and offer evidence based health promoting nursing
interventions for the concern. An evaluation of the concern should be included as if
interventions had been successful.

The nursing care plan can be submitted in a chart (care plan format). See example
posted on Bb.

Process

Select an individual in the community who is willing to meet with you on one or two
occasions to discuss his/her health. This person may be a casual friend, acquaintance,
work colleague, etc. He/she should not be a family member, close personal friend, or
nursing student. Provide a copy of the Information Letter (available on Blackboard) to
your chosen client. This person must sign the Consent to Participate in a Nursing
Assignment (available on Blackboard) prior to the start of the assignment.

The signed consent form is to be submitted to your lab instructor by Feb. 6, 2018.
Include a brief (few sentences) description about your client.

As the first step of a collaborative nursing process, assess/explore health with your
chosen client. Your assessment will include a General Survey (omitting the vital signs),
and a Partial Health History interview including subjective and objective data within
the following Determinants of Health listed in the Mount Royal School of Nursing
Assessment Framework, and covered thus far in Nursing 1112:

 Socioeconomic status
 Social environment and support networks (include ecomap and genogram)
 Education/literacy
 Employment and working conditions
 Physical environment
 Early childhood development
 Personal health practices and coping skills
 Health care services
 Cultural, spiritual, and psychological health
This assessment data will be submitted in chart form as an appendix to the Nursing Care
Plan for analysis by your lab instructor.

As the second step of a collaborative nursing process, you will then analyze the
assessment data for client strengths as well as client concerns. Consider how the
person’s strengths may be used to address the concerns. Do NOT select a medical
diagnosis as a health concern. The concerns must arise out of the assessment data.
From this data, identify ONE priority concern (with rationale) and develop a STROMMC
goal for the individual.

As the third step of a collaborative nursing process, propose possible health promoting
nursing interventions that a nurse might implement to address this one concern and
assist the client to meet the STROMMC goal. High quality source (evidence based)
interventions should be recommended for intervention.

(These interventions are NOT to be implemented on your individual).

A final column in the nursing care plan will include an evaluation of the client outcome
after the proposed intervention (ie) what change would you expect to see in your
client if the health promotion interventions were successful.

A reference list should be included for the evidence based nursing interventions
recommended.
