Instructions:

 This form is to be filled by concerned student himself/herself who is intending to register course(s).
 The student has to submit the form at concerned department’s relevant office before the deadline of registration. Late
submissions of registration forms will not be entertained.
 Registration for improvement can be done for courses with F, W, D, D+, C, C-, and C+ grades only.
 Student concerned will be responsible for any misinformation provided in this form.
 A student may register for a maximum of 6 credit hours during Summer semester. During a regular semester
(Fall/Spring) a student may re-register for a maximum of 4 credit hours (in addition to the prescribed courses).
Student has to attach his/her latest transcript and original Department’s copy of fee payment receipt.
 Separate registration form shall be used for registering courses at department other than the parent department, but
the form has to be submitted with the Semester Coordinator/CMS Operator of the parent department for onward
processing.
 Registration fee once deposited is not refundable, adjustable or transferable.
