Title: To conduct Barangay Nutrition Action Plan CY 2017

RATIONALE:

The Barangay Nutrition Scholars (BNS) of the Municipal of Marihatag will be


conducting a workshop for the Formulation of Action Plan on October 24-30, 2017 @ MNAO
Office Poblacion, Marihatag, Surigao del Sur.

In this connection, we would like to conduct the said activity which is intended to equip
and enhance the capacity of the Barangay Nutrition Scholars in the formulation of their Annual
Action Plan for the smooth implementation of the Program.

OBJECTIVES:

A. It intends to equip and enhance the capacity of the BNS on how to use the Height Board
for the Children Aged 0-71mos. and to fill up the New Forms.

B. To provide needed supplies.

Budgetary Requirements:

- Meals and Snacks P 250.00 x 28 Pax x 2days (P 14,000.00)

Expected Output

1. Barangay Nutrition Scholars are able to fill-up forms & trained to use the Height
Board.
2. Barangay Nutrition Scholars provided with the needed supplies.
