    I, ________________________, age, status, Filipino, and a resident of


    Barangay _____________, Bongao, Tawi-Tawi, under oath depose and state:

    PRELIMINARY STATEMENT

    The person examining me is AHMAD DEEDATT L. KALBIT, Special


    Investigator I, of the Regional Human Rights Commission, Tawi-Tawi Human
    Rights Monitoring Center, with address at DepEd Compound, Tubig Mampallam,
    Bongao, Tawi-Tawi. The examination is being held at my above-stated residence.
    I am answering her questions fully conscious that I do under oath.

    Q : Madam witness, why are you here in THRMC office?

    Q : will you please narrate to me what really transpired on Date?

    “A”, Cerficate of Live Birth as Exhibit “B”. Do you confirm my action?
