I am writing to request for the termination of service of my line under
subscriber -------------------------------------, with landline number ----------------- and
account number --------------------------.
The reason for my request is that I am closing this Internet Shop Branch.
After reviewing my account status I found out that my line subscribed
mentioned above is now outside of the LOCK – IN – PERIOD.
Attached to this letter is my Driver’s License issued by the Land
Transportation Office Baguio City with license number ________, should
