Data Intake Form Barcoding
1 This workbook is to be used for entering collection data for CTFS/SIGEO plots.
The workbook contains three main sections for recording data:
- Collection Data
- Tissue Sample Data
- Geographic Data
These sections can be accessed using the name box in the upper left corner of the spreadsheet. In this spreadsheet
(Instructions and Glossary-the spreasheet you are reading) the name box is directly above "Inst" in the word "Instructions"
and says something like "A2" or "F13".
Once you have clicked on the arrow, the name box will appear. It looks like the image below:
When you select one of the three sections for recording data, the beginning of the section you choose will
appear on the screen and it will be highlighted.
2 The first five rows of the spreadsheet (in blue) are sample data to be used as a guide.
The required fields are shaded yellow and include:
1. Tree Tag No. 9. Collection Date
2. Sample Number 10. Voucher Specimen?
3. Genus 11. Plate No.
4. Species 12. Row Letter
5. Author 13. Column No.
6. Family 14. Tissue Type
7. Collector Number
8. Collector (Team)
3 Each row in the spreadsheet corresponds with a well location within a plate. If tissue from a
single plant is put into more than one well, please use a new row for entering the data.
4 A glossary of field definitions is provided below. Many columns have "comments" with field definitions,
formatting rules, and character limits. A column with comments will have a red triangle in the upper right corner. ---->
To view a comment for a particular column heading, hold your mouse over the header cell until the comment appears.
Please read the comments and use the format shown. Some columns are designed to accept data only in the format
shown in the comment box, for example, all dates must be entered as either mm/dd/yy or dd-mmm-yy.
5 To expedite data entry, we suggest using the "fill" function (Ctrl+ D) when entering geographic data.
6 If you have additional information that you would like to record and it does not fit into any of the existing
fields, you may add this information to the "Notes" fields provided in each section.

Glossary of field names
Collection Data
Tree Tag No. A unique number used to identify a plant within the field plot
Genus The generic name
Species The species name
Below Species Rank Rank below species name, Examples: subsp., var.
Below Species Epithet Name below the specific epithet
Author The authority
Collector Number The expedition leader or primary collector's tracking number. This number is the same for all individuals in a species.
Linked to Tissue Sample and Geographic Data Worksheets.
Sample Number Unique number within the species collected. This number, along with the Collector No. creates a unique identifier.
Collector Expedition leader/primary collector whose primary numbering system being used
Collector (Team) The name of the collector(s) other than expedition leader (staff or team-member)
Collection Date Date sample was collected. Enter in mm/dd/yy format or dd-mmm-yy, mmm is a 3-letter abbreviation for the month
Species Field Code 6-letter code used for CTFS plot species: first 4 letters of genus name, first 2 letters of species name

Order Order
Genus Hybrid Enter hybrid name
Species Hybrid Enter hybrid name
Herbarium Accession No./Catalog No. Accession or catalog number assigned by the herbarium holding the primary voucher specimen collected for this plant
Herbarium Acronym Acronym for the herbarium holding the voucher for this plant - enter in CAPS
Other ID No. Any other identifying number or code used to track this sample
Qualified Identification? Qualifier or certainty of identification. Options include aff., or cf.
Taxonomic Determiner The person who made the original taxonomic determination for this specimen
Date Determined Date the original taxonomic determination was made
Taxonomic Determiner e-mail Contact e-mail for the person who originally determined the specimen
Common Name Common name(s) for the specimen
Cultivated? Is this a cultivated plant? Pull-down: Yes or No
Cultivated Name If above is yes, then enter the name for the cultivated plant here
Habitat The plant habitat
Habit Pull-down: tree, herb, shrub, liana/vine, epiphyte, other
Plant Description 1 Plant description. 250 character limit. Please enter Habitat and Habit information in the fields
provided
Plant Description 2 If Plant Description 1 is more than 250 characters, continue the description here
Taxonomic Notes/Other Specimen Details Additional information related to the collection, identification, and taxonomy of this sample
Row Letter The row letter on the 96-well extraction plate where the tissue sample is deposited
Column No. The column number on the 96-well extraction plate where the tissue sample is deposited
Tissue Type Type of tissue collected for the DNA extraction. Pull-down menu: leaf, fruit, seed, root, bark, cambium, other.
Geographic Data
Plot Elevation Elevation of the research plot (not of the specific sample or individual). Enter in meters, a range is ok.
Plot Latitude Latitude of the research plot (not of the specific sample).
Enter in degrees, minutes, seconds, with a space between each (dd mm ss).
Plot Longitude Longitude of the research plot (not of the specific sample).
Enter in degrees, minutes, seconds, with a space between each (dd mm ss).
