    AFFIDAVIT OF LOSS

    I, CYNERSON P. DEMECILLO, of legal age, Filipino, single and a resident of #19 Mindanao St.
    Phase 4 Payatas P, Quezon City, after having been duly sworn to in accordance with law, I hereby depose
    and state:

    1. I am the owner of a Driver’s License with no. PDL N04-13-010964 issued by the Land
    Transportation Office.
    2. However, on March 13, 2017, I was a victim of carnapping and inside the car was the
    driver’s license.
    3. Diligent efforts was exerted, however, the same is now beyond recovery;
    4. As such, I am executing this Affidavit of Loss to attest to the truth of the foregoing facts
    and to support my application for the issuance of another Driver’s License ID, in lieu of the
    ID that was lost and beyond recovery.

    IN WITNESS WHEREOF, I have hereunto set my hand this 9th day of May, 2017 in Quezon City,
    Metro Manila, Philippines.
