    AFFIDAVIT OF NO INCOME

    I, FELOMINO N. LOMARDA, Filipino, of legal age, married and


    a resident of Yanangan, Loay, Bohol, Philippines, after having
    been duly sworn to in accordance with law, hereby, depose and
    say:

    1. That, I am the husband of MARIA VISITACION B. LOMARDA,


    who is an employee at the Bohol Community Multi-Purpose
    Cooperative (BCMPC), Poblacion Ubos, Loay, Bohol, Philippines;

    2. That, I, at present am not anymore employed and not


    engaged in any gainful business/occupation, and therefore I have
    no more income from self-employment;

    3. That I execute this affidavit to attest to the truth of


    the foregoing fact, and to support for the requirements at the
    Bureau of Internal Revenue (BIR), Tagbilaran City, and for
    whatever legal intents and purposes this may serve.
