Scaffolding Collapse (Z) : Moved To Another Location Inadequate Design of Scaffold Breakdown of Support

The scaffolding collapsed due to a combination of factors including: inadequate design of the scaffold which was not properly tied to the structure and used tubes of different sizes; failure to wear personal fall arrest systems; and excessive impact loading on the platform from materials placed on it. The collapse resulted in the worker being injured when they fell after the platform gave way.
