Procedures Performance Criteria

1. Greetings and introduction of self
2. Inform consent
3. ASSESSMENT AND EVALUATION: vital signs (BP), limb girth measurement
4. PRECAUTIONS: impaired sensation or mentation, uncontrolled HTN, cancer, stroke, PNI
5. CONTRAINDICATIONS: CHF, pulmonary edema, DVT, obstructed lymphatics, arterial
PVD, skin infection
6. INTRODUCTION OF PT PLAN: INTERMITTENT PNEUMATIC COMPRESSION
PUMP
7. RATIONALE: control edema & scarring, promote healing of venous ulcers, shaping of
residual limb
8. MATERIALS: Intermittent pneumatic compression unit, sleeves, stockinette, BP app.
9. DEMONSTRATION BY PT
10. DOSAGE
A. INFLAT/DEFLAT TIMES (3:1): 80-100s/25-50s (edema); 40-60s/10-15s (residual limb)
B. INFLATION PRESSURE: 30-80mmHg; just below patient's diastolic BP; lower for UE
than LE
C. DURATION: 1-4 hours /treatment session
11. INSTRUCTION: call out if concerned esp. with dizziness, nausea; discontinue with BP
12. PATIENT POSITION: comfortable, relaxed, supported
13. PT STANCE
14. EXECUTION
1) Check wirings and machine. Plug machine and test if functional
2) Measure and record the patient's blood pressure
3) Position patient optimally in sitting or supine
4) Remove clothing and jewelry from area to be treated
5) Apply foam/cotton padding around anatomical indentations; dress & cover wound
6) Place the affected limb in an elevated position
7) Place a stocking over area to be treated & smooth out wrinkles
Apply the sleeve over the stockinette
9) Attach the hose from the pneumatic compression pump to the sleeve
10) Set appropriate compression parameters: inflation/deflation times, pressure
11) Monitor patient throughout treatment duration - BP monitoring
12) At end of Rx, apply compression garment/bandage to maintain in edema

3 A. PRECAUTIONS: impaired thermal sensation, infection, impaired cognition, recent skin
graft
4 B. CONTRAINDICATIONS: maceration around wound, bleeding
5 INTRODUCTION OF PT PLAN: WHIRLPOOL
6 RATIONALE: superficial cooling or heating, wound care, water exercise, pain control
7 MATERIALS: whirlpool tank, towels, thermometer, rags, mats
8 DEMONSTRATION BY PT
9 DOSAGE
10 A. INTENSITY: mild, comfortable warmth (HEAT)
11 B. DURATION: 10-20 mins
12 C. FREQUENCY: as prescribed
13 INSTRUCTION: avoid pressure of limb on edge of pool, avoid touching the turbine, too
hot
14 PATIENT POSITION AND DRAPING: comfortable, supported
15 PT STANCE
16 1) Fill the tank with water, 3/4 full
17 2) Check wirings, turbine/agitator, plug whirlpool and check if functional
18 3) Select appropriate temperature range by heating or putting ice on water
19 4) Allow px to undress area to be treated, remove jewelries & provide gown
20 5) Clean the part to be immersed
21 6) Position px comfortably, place towel on rim of tank to distribute pressure
22 7) Immerse in water the area/part to be treated
23 Adjust the height, direction and aeration of the turbine
24 9) Turn on the turbine
25 10) Instruct patient on movement of part desired
26 11) Monitor patient throughout treatment duration of 20 mins
27 AFTERCARE
