fees are nonrefundable. Please provide a packet including each entry form submitted along with ONE check for the total
of all entries.

Deadline is January 15th, 2016. Entries may be dropped off at New Haven Print, 7531 US 930 East, New Haven, IN.
Work may be entered for more than one category. Separate form and entry fee must be submitted per category.Work must
have been created between Jan. 1 and Dec. 31, 2015.

Prizes: Silver, Gold, Best of Show, Peoples Choice, and Judges Choice

Additional Information:
Each entry should go into an envelope (or box for three dimensional entries) to protect them during transport. Securely
enclose a completed entry form with each entry. Fill out the entry form completely, as this information will be used if you
are a winner. Be sure to credit all persons responsible for the project. If this is a student entry, be sure to include your
school name, school location, and school website. Also include the name of instructors who advised you. If applicable,
you may also provide a personal website. We cannot return any entry materials.

For Audio/Video/Web/App Entries please provide a data disk for use at judging and include a screen shot or image to use
for the Winners book.
