Students Survey Questionnaire

The researcher is a BSEd student major in Mathematics at Carmelite College who is


in the process of writing her research paper entitled Difficulties in Solving Mathematical
Problems among Junior High School Students in the selected public and private schools in
the Municipality of Siquijor. The enclosed questionnaire has been designed to collect
information about the demographic profile of the respondent and their difficulties in solving
mathematical word problems. Your participation in this research will be a great help for the
realization of its purpose. Your responses will remain confidential and anonymous. No one
other than the researcher herself will know your answers to these questionnaires.

he returned by bus at 90 km/hr. if his total traveling time was 5 hours, how long was his trip
by car?
4. Number Problem: The sum of two numbers is 45. Four times the lesser is 20 less than
the greater. Find the numbers.

5. Mixture Problem: Solution A s 2% alcohol and solution B is 8% alcohol. A drugstore


owner wants to mix them in order to get a 60-liter solution that is 3.2% alcohol. How many
liters of each should the owner use?
