The effectiveness of EAM / CMMS system can be measured not only by how well it

helps to monitor MTBF & MTTR, but also how well it provides insights to increase

MTBF & reduce MTTR. One of the best practice approach to increase MTBF is to

perform Root Cause Analysis (RCA) for the top contributors of Asset failures and

implement corrective actions . This article explains how such RCA can be performed

using Maximo EAM/CMMS system.

The Asset failures can be captured systematically using Failure codes (Problem,

Causes and Remedy) in Reactive Work Orders of Maximo . The Causes and

Remedies reported in the Work Order are symptomatic. Therefore, detailed analysis is

required to identify the root causes and the corrective actions. A well-defined failure

code hierarchy and well-structured failure reports are required to perform such root

cause analysis.

Failure Hierarchy

Its imperative to have well-defined failure hierarchy to perform RCA. The failure codes

should illustrate the failure clearly and should be consistent and searchable. The below

diagram illustrates the key factor to be considered in defining the failure hierarchy:
ISO14224 helps to define standardized Failure codes. The Failure code library of

ISO14224 can be modified to suit individual organizations requirement. Though

ISO14224 guidelines are for process industry Assets, the examples will help to model

the failure hierarchy for other Assets. Please refer the below spreadsheet for the list of

Problem, Cause and Remedy code for various Asset types, as extracted from

ISO14224.

Failure Reports

A well-structured failure report helps to identify potential areas to focus and the actions

required to improve the MTBF. The report should list down the failure details at a Unit -

Asset type level. The report should also highlight top 5 or 10 problem codes based on

the number of failures for a specific time period.


Root Cause Analysis (RCA) & Implementation

Root Cause Analysis can be performed based on the Failure reports from Maximo. It

involves the following six steps:

One of the biggest challenge / prerequisite to perform RCA is to capture failure codes

as part of the work orders. This challenge can be addressed by using an intuitive

Mobile app for entering the failure codes as part of work reporting. The following image

from a Maximo Mobile app illustrates the quick and easy way to enter failures codes for

a reactive work order.


Conclusion

The Root Cause Analysis & Implementation eliminates frequent failures and thereby

improves MTBF. A well-thought-out plan is essential to analyze the failure codes from

Maximo on a periodic interval to perform RCA. A formal committee can be setup to

conduct meetings and devise action plan to leverage continuous improvement

opportunities. If Corrective and Preventive Action (CAPA) team exists, they can be

entrusted with this activity.
