Richard Macabinlar has been suspended for two days for two reasons: 1) Multiple errors occurred in client refunds and changes due to his failure to review, update, and balance client account ledgers before transactions. This cost the company money. 2) He released a loan to a client without approval from his manager, despite being refused. He has been instructed before on the need to review accounts and get manager approval for loans. He must now submit an explanation letter for these issues within 48 hours.

Failure to Review, Update and Balance the Account Ledgers of Clients/ Errors in the Release of
Change/Refund

The latest audit report revealed that there have been multiple errors in the release and posting of the change/refund
of several clients due to failure on your part to review, update and balance their account ledgers prior to the
transaction. These errors proved disadvantageous to the company since the amount released to the clients is more
than what is due them.

Further investigation have shown that you have already been instructed by your manager to include, as part of your
standard procedure, the reviewing, updating and balancing of the account ledgers of your clients prior to the conduct
of any transaction, but obviously missed.

Release of Loan Without the Approval from the Manager

It has reached to the management that you have caused the release of loan to a certain client without the approval of
your manager. And despite the clear refusal by your manager, you still released the disapproved application. Please
know that the decision of your manager must be always regarded with due respect as he has the command
responsibility with regard to all the operations in your office, specially the approval of loans and to any all related
transactions. You must know that releasing of loan without the approval of manager is considered as
insubordination.

With all of these in present, you are hereby exacted the penalty of two (2) days suspension for failure to review,
update and balance the account ledgers of your clients prior to the conduct of any transaction and for simple
insubordination in failing to follow to the instruction of your manager.

It is hoped that this sanction will motivate and prompt you to exert more effort and deliver satisfactory performance
of your duties and responsibilities with minimal supervision from the management.

Finally, in the light of fair dealing and impartiality, you are directed to make an explanation letter regarding the
aforementioned matter. Please submit the same to our HR Personnel Officer within 48 hours from the receipt of this
memo.
