CONDUCT COMPETENCY ASSESSMENT SCRIPT

1. CHECKING OF THE LABORATORY


The assessor will check the laboratorys tools and materials as stipulated in the
Training Regulation.
A: Good morning sir, I have now checked the laboratory as well as the facilities, tools and
equipment needed in Computer Systems Servicing NC II and it complies with the standards
prescribed by the Training Regulation.

2. CANDIDATES WILL ENTER THE LABORATORY


The assessor will open the door so that the candidates will enter inside the laboratory.

3. OPENING PRAYER
A: Good afternoon everyone, please stand for an opening prayer.

4. INTRODUCE MYSELF
A: Once again, good afternoon everyone. Im Darell Kim P. Cervantes, your competency
assessor for Computer System Servicing NC II.

5. ACKNOWLEDGE THE PERSONNEL


A: And I would like to acknowledge the presence of ________________________________. Good
afternoon Maam/Sir. You dont have to worry about their presence, they are here to oversee
the conduct of assessment and to check if the conduct of assessment is in accordance with
the prescribe competency assessment, methodology and procedures.

6. ASSESSMENT PACKAGE:
A: By the way, I have here the complete Assessment Package given to me by the TESDA
Representative which consists of the following:

Self-Assessment Guide

Specific Instruction to the Candidate

7. SUBMISSION OF ADMISSION SLIP
A: Before we start, kindly submit your admission slip.
C: (They will submit the admission slip.)

8. CHECKING OF ATTENDANCE
A: Before we proceed, please acknowledge by raising your hand as I called your name, and
please sign the attendance sheet provided to you.
C: (They will sign the attendance sheet.)

9. PRESENT THE ACCOMPLISH APPLICATION FORM & SELF-ASSESSMENT GUIDE


A: I have here with me your accomplished application form and self-assessment guide. This
is an indication that you know and can perform all the units of competency enumerated in
Computer Systems Servicing NC II.

10. WELCOMING IN THE CENTER


A: By the way, I would like to welcome you all in our center for your assessment in Computer
Systems Servicing NC II.

11. PURPOSE OF ASSESSMENT


A: The purpose of this assessment is to measure your knowledge, skills and attitudes as
well as to determine the level of your competency.

12. ASSESSMENT BASED ON 4 DIMENSIONS OF COMPETENCY


A: The assessment is also based on the four dimensions of competency- the Task Skills,
Task Management Skills, Contingency Management Skills and Job/Role Environment
Skills.

13. UNITS OF COMPETENCY


A: Today, I am going to administer competency assessment in CSS NC II. This qualification
is composed of 3 competencies- the Basic, Common and Core.

14. CORE COMPETENCIES


A: For the core competencies, we do have:
Prepare the dining area/restaurant for service
Welcome guests and take food and beverage orders
Promote Food and Beverage Products

Provide food and beverage services to guest
Provide Room service
Receive and Handle Guest Concerns

15. TASK TO BE PERFORMED


A: The different tasks to be performed are the following:
1. Install computer systems and networks
2. Sett-Up Computer Networks
3. Set-Up Server
4. Maintain and repair computer systems and networks

16. EVIDENCES OF THE ASSESSMENT


A: To gather evidences in the assessment, we will utilize Oral Questioning, Demonstration
and Written Test.
17. ASSESSMENT METHODS
A: The assessment methods we are going to used are Oral Questioning, Demonstration and
Written Test. These assessment methods will measure your underpinning knowledge and
skills in Computer System Servicing.

18. PERSONAL NEEDS OF CANDIDATES


A: If you have personal needs, the lavatory is located near the hallway as well as the comfort
room. If you want to take your meal, the canteen is located near the library. However, during
the assessment, you are no longer allowed to go out from the assessment room.

19. ALLOWABLE ADJUSTMENTS


A: If you have difficulty understanding the given instructions as well as in the written test,
please dont hesitate to ask my assistance because I am willing to assist you.

20. DOS & DONTS DURING ASSESSMENT


A: Also, if you have other concerns, questions or clarifications during the assessment, please
raise your hand so that you will be recognized and given appropriate assistance, however,
during the assessment, please refrain from asking questions, it is no longer allowed. If you
have personal needs, please ask my permission so that we can address your need
immediately.

21. RIGHTS TO APPEAL

A: If you have doubts or questions with the assessment result, you have the right to appeal.
Just submit a letter to the TESDA Provincial Office where the center and the assessor are
accredited.

22. ISSUANCE OF NC/COC


A: For the issuance of National Certificate, the AC Manager will inform you where to get
your certificate in 5 working days after the assessment.

23. REASSESSMENT PROCEDURES


A: After the assessment if you found to be not yet competent, dont worry because you can
be reassess one month after this assessment. If your reassessment is full qualification, then
you are going to undergo the entire assessment procedures, but if found that you are
competent in some competencies, then your reassessment will only focus on the
competencies which you are not yet competent. Am I understood?
C: Yes, Sir!

24. OCCUPATIONAL SAFETY AND HEALTH REQUIREMENTS (OSH)


A: During the assessment, please observe safety. Our first aid kit is located near the door.
Proper use of Personal Protective Equipment must also be observe. Please observe proper
body mechanics when lifting breakable materials. In case of unexpected fire, we have fire
extinguishers installed inside the room. Our fire exit is also properly labelled. If you
encounter any untoward incidents during the assessment, please call my attention so that
we can immediately give the appropriate first aid.

25. CONFIDENTIALITY OF ASSESSMENT OUTCOMES


A: With regards to the assessment results, we observed strict confidentiality. The
information of your assessment result is only available to the authorize personnel and to
the institution where you have your actual training. In relation to this, the assessment
package is kept and secured. This is to ensure the integrity of the assessment package. In
line with this, may I ask the candidates to please refrain from giving any form of information
taking place during the assessment procedures. Am I understood?
C: Yes, Sir!
A: Do you have further questions or clarifications?
C: None Sir!

26. DISTRIBUTION OF THE SPECIFIC INSTRUCTION TO THE CANDIDATES
A: So now, I will distribute the specific instruction. Please read and understand it carefully.
If you have difficulty, I am willing to translate it in our local dialect if you want.
C: (You will read the specific instruction)
A: Do you have further clarification or questions?
C: None Sir!

27. ASSIGNING OF WORKING STATION


A: Before you proceed to the practical work area, let me assign you first to the different
working stations. For Ms. _______________________ you are assigned in Working Station #1
where you are going to perform _________________; Ms. ___________________________, you are
assigned in Working Station #2 where you are going to perform ___________________; and
Ms. _______________________, you are assigned in Working Station #3 where you are going
to perform ______________________. By the way, let me remind you that you will be given 2
Hours to accomplish the entire tasks of the assessment.
A: Now, everybody proceed to the practical work area.

28. PROCEED TO THE PRACTICAL WORK AREA


A: Okey, on my signal you may start the task.
A: Candidates, you may begin!

29. AFTER THE TIME


A: Candidates, your time is up. Kindly proceed to the waiting area and wait for your turn
for the oral questioning.

30. ORAL QUESTIONING/INTERVIEW


A: Ms. __________________, kindly follow me.
A: Please take a sit Ms. ______________________.
A: What is the importance of 3-minute check?
C: The importance of 3-minute check Sir, is to ensure guest satisfaction.
A: Thank you Ms. ________________. You can now return to your seat.

A: Okey, in case someone has a complaint with the service you have, how are you going to
handle it?
C: (Answer in nervously and stammering) Ah! I will . I will. Ahh.. I will ask the guest problem.
I will ask his/her question to the service.
A: Is that all Ms. __________________.
C: Yes, Sir. Thats all.
A: Okey, Thank you Ms. ________________. You can now return to your seat.

A: Ms. ______________________, its your turn.


A: Please take a sit.
A: During the service, who do think must be served first, children or ladies? Why?
C: Children must be served first because they are more active which might cause
disturbance to others if not catered first.
A: Okey! Thank you Ms. ____________________. You can now return to your seat.

(ASSESSOR WILL RECORD THE RESULTS)

31. FEEDBACKING
A: (Call the candidate one-by-one.)
A: Ms. _________________________. Please take a sit.
A: How do you feel about the assessment?
C: During the assessment Sir, I feel a little bit relax because I was been able to perform the
task properly and correctly. I am also confident that I answered the question correctly.
A: Thats true Ms. ____________________. Your opinion coincides my observation. You really
have the knowledge and skill in performing the task given to you. You have skills in
Installing Computer Hardware and Setting Up Computer Networks . You have also answered
the questions correctly. However, you did not observed proper procedure in assembling the
motherboard, but still you properly performed the task. Nevertheless, it doesnt affect your
overall performance. With that, I would say that you are competent in this qualification.
Congratulations!
A: For the issuance of National Certificate, the AC Manager will inform you where to get
your certificate in 5 working days after the assessment. Upon claiming, please secured 1

pc. Passport Size ID picture with white background; 2 pcs. Documentary Stamp and 100
pesos for the Certification Fee.
A: With that, please sign the Competency Assessment Results Summary.
C: (You will sign the CARS.) Detached the candidates copy.
A: Do you have further questions Ms. ____________________?
C: None Sir. I have no more questions.
A: Okey, Thank you very much and once again CONGRATULATIONS!

A: Please take a sit Ms. _____________________.
A: How do you feel about the assessment?
C: Honestly Sir, during the assessment I feel quite nervous. I did not even perform the Table
Set-up correctly because of my tense. During the interview, I was also tense because I was
not sure about my answer thats why I stammered as I answer the question.
A: Really? So what will you do if you will be asked to perform the same task next time?
C: I will have more practice in order to acquire the needed skill. I will also make some review
so that I can answer the questions correctly.
A: Thats good. If you will do that, surely you will become competent in this qualification.
Actually, you already have some skills. In fact, you have the skill in handling the tools and
materials. You also have the skill in Installation of Computer Hardware. However, you need
more practice in Setting up Server. During the interview, I feel your tense thats why you
did not answer the question correctly. So based on your performance, I would say that you
are not yet competent in some of the competencies in this qualification.
A: If you have doubts with the assessment results, you have the right to appeal. Just submit
a letter to the TESDA Provincial Office where the center and the assessor are accredited.
A: In same manner, you can still be reassess, one month after this assessment.
A: Okey, so please sign the Competency Assessment Results Summary.
C: (You will sign the CARS.) Detached the candidates copy
A: Do you have further questions Ms. _______________________.
C: None Sir. I have no further questions.
A: Okey, Thank you very much Ms.__________________________.

(THIRD CANDIDATE will just dramatized the signing and detaching of CARS.)

(ASSESSOR WILL RECORD THE ASSESSMENT OUTCOMES.)

32. ASSESSMENT REPORTS WILL BE SUBMITTED TO AC MANAGER


A: The assessment reports will be given to the AC Manager. Afterwards, the AC Manager
will give it to the Processing Officer to process and accomplish the Registry of Workers
Assessed and Certified (RWAC). The Processing Officer will sign the RWAC and other reports.
Then, this will be given back to the Assessor and AC Manager to sign the reports and other
documents. Then, the assessment package with the accomplished reports and other
documents will be given back to the TESDA Representative.

A: I am done now in my CCA Maam/Sir.
