This is how I complete the UCC3 Amendment to add collateral (i.e., Bonds, Notes,
Letters of Credit, etc.) to initial Account/Case AFV UCC1.
(Box B - Acknowledgment Sent To)
Put the Strawparty name here.
(Box 1a - Initial Financing Statement #)
Box 1a - Place the initial UCC-1 filing number that was issued when you filed the AFV
on the case here
(Box 6 - Current Record Information)
Box 6a - Place STRAWPARTY name here. (Duplicate what you put on the UCC-1
here)
(Box 8 - Amendment (Collateral Change))
(X) Check "Added" Box only; and then
Box 8 - Type this in:
The following property is Accepted for Value, Exempt from Levy, and is herewith
registered in the Commercial Chamber. Said property is Liened at a collective sum
certain Ten Million USD ($80,000,000.00)and is the private property of the Secured
Party as authorized agent of the Debtor. Property described as:
1. Letter of Credit, No. LOC-09CR12345A-CUF ($40,000,000.00) issued by Chuck-
Eugene: Farley on August 25, 2009 for the purpose of securing the following:
a. Private Bond of Indemnity, No. PBOI-09CR12345A-JUDGE-CUF
($10,000,000.00)
b. Private Bond of Indemnity, No. PBOI-09CR12345A-DA-CUF
($10,000,000.00)

($10,000,000.00) for the purpose of securing the funds to indemnify
indemnitee in the event of loss or injury sustained for performance under the
bond order;

Eugene: Farley on August 25, 2009 in the amount of Ten Million USD
($10,000,000.00) for the purpose of securing the funds to indemnify
indemnitee in the event of loss or injury sustained for performance under the
bond order;
4. Private Bond of Indemnity, No. PBOI-09CR12345A-COURT-CUF issued by
Chuck-Eugene: Farley on August 25, 2009 in the amount of Ten Million USD
($10,000,000.00) for the purpose of securing the funds to indemnify
indemnitee in the event of loss or injury sustained for performance under the
bond order; and,
5. Replacement of Surety Bond, No. ROSB-09CR123456A-cuf-CUF issued by
Chuck-Eugene: Farley on August 25, 2009 in the amount of Ten Million USD
($10,000,000.00) for the purpose of securing the funds for replacing Chuck-
Eugene: Farley, a living man, as Surety for CHUCK EUGENE FARLEY, Ens Legis
Defendant and releasing the corpus therefrom.
Said property is to apply to Account/Case No. 09CR-12345-A Charging Information,
True Bill Indictment, Warrant, and Minute Order(s) filed with the LOS ANGELES
