Dear {Candidates name or All (if sending a bulk email)},

My name is {ENTER YOUR FULL NAME} I am the coordinator for the {ENTER REGIONAL
GROUP} Professional Review Interviews (PRI). I have received your application form, two-
page experience report and your IPD Final Report forms from the Institution HQ. My task is
now to arrange your PRI.

The interviews will be held on {INTERVIEW DATE/S} at the {VENUE NAME}. Address details
and directions to the venue can be found at: {ENTER WEBSITE LINK FOR VENUE}.

Please confirm by email, at your soonest convenience, that you will be able to attend for
interview on the stipulated date/s. The important thing to do is to get the {ENTER INTERVIEW
DATE} date in your diary, if you have not already done so, and to get permission from your
line manager to have leave to attend the Interview! Please note the attached letter giving you
a little more information about the interviews.

IMPORTANT
As you will be aware from your initial application, you are required to submit some additional
documentation to me in hard copy format. One of my responsibilities is to issue this to the
reviewers so that they can prepare for your interview in good time. I trust that you have
already got this ready or that it will take you only little time to put it together.

What I need from you now depends on your Initial Professional Development (IPD) route,
bearing in mind that all three routes require the submission of two copies of a portfolio of work
and this portfolio must not exceed 40 mm depth (single-sided) or 25 mm depth (double-
sided), excluding binding. As stated on your application form (Form M for those of you
applying for Chartered Membership, Form AM for those applying for Associate-Membership
and Form T for those applying for Technician Membership), those of you who have applied via
the:

Accredited Training Scheme Route should send me TWO COPIES of the following:

a). Your relevant Training Records

c). A personal development record in the form of a diary (or similar)

c). Portfolio of work

Individually Managed Route should send me TWO COPIES of the following:

a). Your completed IPD Quarterly Report Forms

b). Your completed Progress Summary Records

c). A personal development record in the form of a diary (or similar)

d). Portfolio of work

Retrospectively Collated IPD route, should send me TWO COPIES of the following:

a). A personal development record in the form of a diary (or similar)

b). Portfolio of work

As I indicated above, given that the deadline for submission was 1 September you should
have prepared the additional documents quoted above in readiness for the interviews.

PLEASE SUBMIT THE DOCUMENTS LISTED ABOVE ON OR BEFORE {ENTER THE


REQUIRED DATE}. This is the FINAL deadline. Im afraid that I will not be able to relax this
deadline for anyone.
All documents must be sent, addressed to {ME OR NAMES OF REVIEWERS}, to: {ENTER
THE RELEVANT ADDRESS/PROVIDE IT SEPERATELY}. If you would prefer to deliver your
submission by hand, {PROVIDE DETAILS OF HOW THEY ARE ABLE TO DO SO}.
