CNL JV SWP Standard 404 Confined Space Entry
1.1 Purpose
The purpose of this Safe Work Practices (SWP) Standard is to ensure that confined space entries
are performed in a safe and controlled manner.
1.2 Objective
This SWP standard establishes requirements for performing confined space entries.
1.3 Scope
This Confined Space Entry, Safe Work Practice (SWP) covers work performed by Chevron
employees and their delegates and contractors within Nigeria Mid-Africa Strategic Business Unit
(NMA SBU) Chevron Nigeria Limited Joint Venture (CNL JV) operational control.
2.0 Requirements
1. Use the definition of confined space to identify confined spaces at any location.
2. Consider all viable alternatives to performing an entry, such as the use of mechanical
tools to perform tank cleaning from outside the confined space before performing
confined space entry.
3. Hazards associated with confined spaces shall be identified and mitigated prior to
beginning work.
4. Gas testing for entry into a confined space must be performed by a qualified gas tester.
5. A rescue plan must be in place before any confined space entry.
Critical: Never attempt to enter a confined space, even in an emergency, until help has
arrived. Dont try to lift a person out of a hole without help.
6. Rescue personnel must be trained and competent and have the ability to perform their
responsibilities. Rescue personnel must also have the correct rescue equipment at the
work location.
7. An Entry Watch must be onsite at all times when personnel are in the confined space.
The Entry Watch must maintain an entry log which includes the name of the entrants
and the time(s) they entered and exited the confined space. The Entry Watch may not
leave his or her position unless a qualified replacement Entry Watch is in place.
8. The confined space must be secured and any flammable gas sources must be removed
whenever the confined space is left unattended for any period of time.

Willingness to rescue on company premises For offsite services, verify that the service is
willing to perform rescues at the designated workplace. CNL JV cannot rely on a rescuer who
declines, for whatever reason, to provide rescue services.
Availability of adequate communications For a timely rescue, it is important that an
adequate method for communication between the Entry Watch and prospective rescuers be
available.
Rescue equipment For rescues into spaces that may pose significant atmospheric hazards
and from which rescue entry, patient packaging and retrieval cannot be safely accomplished in
a relatively short time (15 to 20 minutes), consider using air supplying respirators (with escape
cylinders) for the rescuers and supplying rescue air to the patient. If the decision is to use a
SCBA, ensure that the prospective rescue service has an ample supply of replacement cylinders
and that the procedures for rescuers to enter and exit (or be retrieved) fall well within the
SCBAs air supply limits.
Space entry and elevated rescue If the space has a vertical entry greater than five (5) feet
(1.524 meter) in depth, determine if the prospective rescue team or service can properly
perform entry rescues. Furthermore, check if the team or service has the technical knowledge
and equipment to perform rope work or elevated rescue, if needed.
Necessary skills It is imperative that the rescue team or service has the necessary skills in
medical evaluation, patient packaging and emergency response.
Necessary equipment Ensure that the rescue team or service has the necessary equipment to
perform confined space entry rescues.

Attend a pre-work safety briefing conducted by the Work Team Leader (WTL).
Know the chemical or toxic hazards that may be encountered during entry, including the signs and
symptoms of exposure.
Review the Emergency Action Plan before entry. (Rescue Plan, refer to Section 5.8)
Wear the protective clothing and respiratory protection specified on the Confined Space Entry Permit or
by the Work Team Leader (WTL).
Sign the Confined Space Entrant Log when entering the confined space.
Check with the Safety (Entry) Watch when entering and exiting the confined space.
Alert the Safety (Entry) Watch whenever they feel the effects of overexposure, or when a hazardous
condition develops.
Exit the confined space as quickly as possible whenever the Safety (Entry) Watch gives an order to
evacuate, the evacuation alarm is activated, or when a hazardous condition develops.
Complete a General Work Permit (GWP) for the preparation work, check the precautions and
restriction requirements, and give the GWP to the Work Team Leader (WTL).
Review the GWP with the Work Team Leader to ensure that there is a complete understanding as to the
scope of work and the possible hazards that could be encountered.
Prepare a drawing/schematic of the confined space that indicates the location and type of isolation
measures. Sign and date drawing/schematic and attach it to the GWP.
Enter the Date, Time, GWP No, Control No, and Area Emergency Response No on the Confined Space
Entry Permit; and specify the gas testing and classification of the entry that must be conducted and the
personnel and safety equipment that will be required. Give the Permit to the Work Team Leader.
Together with the Work Team Leader, develop the Emergency Action (Rescue) Plan.
Ensure that the designated confined space has been properly isolated and that power sources are locked
and tagged, or have been electrically disconnected, or fuses have been pulled, when required. Make
sure the equipment has been tied off as necessary to stop any motion.
Approve the initial opening of the designated confined space after it has been properly emptied,
depressurized, and isolated.

Make sure that a Qualified (Certified) Gas Tester has conducted the initial gas tests, and ensure that the
results are acceptable for entry without supplied breathing air. If test results are not acceptable:
1. The confined space must be cleaned and/or ventilated longer; or
2. Breathing air equipment must be used by anyone who enters the confined space. If breathing air
equipment is required, the box indicating such must be checked on the Permit.
Review the permit preparation requirements with the Site Checker (Operator/Facility Representative)
and the Work Team Leader to ensure that they have been completed.
Ensure that the Work Team Leader has assigned both an Entry (Safety) Watch and a Backup Person if
breathing air equipment is required.
When all requirements are met and the confined space is safe to enter, sign the confined space entry
permit.
Review the permit conditions with the Area Controller (Field/Area Supervisor). If the specified
condition are satisfactory, Area Controller to sign in Section E of the permit.
Periodically observe the job and stop work if an unsafe condition develops.
After the job is complete, inspect the confined space with the Area Controller and or Site Checker and
the Work Team Leader to make sure it is ready to close. Others, such as the Project Engineer or
Equipment Inspector, may be required to check the confined space and give approval before it can be
closed.
Wear a Yellow/Orange color vest, so he/she can be easily recognized as being an Entry (Safety) Watch.
Remain immediately outside the opening to the confined space when workers are inside, maintaining
visual contact and communications with them at all times. Communication may be by hand signals,
radio, or megaphone.
Have a two (2)-way radio tuned to the appropriate channel/frequency for communication with others.
Make sure the Emergency Action (Rescue) Plan is at the job site and available for review by the
workers.
Ensure that no one removes blinds or shuts off ventilators or other equipment necessary for the safety of
workers inside.
Keep track of workers when they enter and leave the confined space to make certain everyone is
accounted for. This includes:
1. Recording the times the workers enter and exit the space; and
2. Crossing out a workers name whenever he/she leaves the confined space and will not be re-
entering.

Ensure that SCBA (self-contained breathing apparatus) is immediately available outside the confined
space for rescue, and be trained in its use.
If hot work is being done inside the confined space and the Entry Watch is doubling as a Fire Watch,
he/she must have a 30-pound dry chemical fire extinguisher immediately at hand, as well as, a charged
firewater hose, when available.
Be alert for changing conditions and hazards that might affect the safety of workers inside the confined
space.
Call for assistance in case an emergency develops inside the confined space.
Do not enter the confined space until another worker outside has assumed Entry (Safety) Watch duties.
If entry is for rescue and the emergency is the result of an unsafe atmosphere, SCBA (self-contained
breathing apparatus) must be used.
Review the General Work Permit with the Entry Supervisor (Person In Charge) and the Work Team
Leader.
Ensure that the confined space has been properly isolated, power sources locked and tagged out or
electrically disconnected or fuses removed when required, and equipment tied off or blocked as
necessary to prevent any motion.
Hang Danger Do Not Enter tags on all openings at the time the equipment is opened.
After confirming that the confined space is safe for entry (with or without breathing air equipment),
return the permits to the Entry Supervisor and the Work Team Leader for final approval of entry.
Install the approved Permits on the primary opening where the confined space is to be entered, and
remove the Danger Do Not Enter tag from the opening.
Note: Danger Do Not Enter tags should be left on all openings that are not to be used for entry.
Verify that a Entry (Safety) Watch is at the site and that a Backup Person is immediately available if
workers inside the confined space are using breathing air equipment.
Ensure that any time the confined space is open, but not approved for entry, a Danger Do Not Enter
tag is hung at each opening. This includes the following circumstances:
1. When conditions become unsafe and workers must leave the confined space,
2. Whenever any of the permits have been removed, and
3. At the end of the day, if the confined space is still open.

Check to make sure that follow-up gas tests are being taken as required. If hot work is taking place
inside the confined space gas testing will be continuous, gas tests results shall be logged every 30
minutes in the Gas Test Log section of the confined space entry permit.
After the job has been completed, make an inspection with the Entry Supervisor, Work Team Leader
and others (i.e., Site Checker) as necessary, to ensure that it is safe to close the confined space.
After the confined space has been closed and secured, ensure that the area is cleaned up and tools and
equipment have been removed from the site.
Ensure that blinds are pulled, lines are reconnected and the Blind List is completed.
Backup Person (Required whenever breathing air equipment is required for entry.)
Remain in the area of the confined space, and be immediately available for assistance upon request of
the Entry (Safety) Watch.
Have a separate air supply available, such as SCBA (self-contained breathing apparatus), and be ready
to don it in case of an emergency.
Monitor the air supply for the workers inside the confined space (if the air supply is located
immediately outside the space).
