PARENTAL CONSENT FORM

The following minor: ______________________________________ has my permission

to be transported by LIMO4ME.COM on ____/____/_____. This includes, but not


limited to, transportation to and, or from any school related event.

I will be responsible for my minor child, including, but not limited to, the behavior of my
minor child.

I agree that no alcoholic beverages or any other prohibited substances will be brought
into the vehicle by my minor child.

Im fully aware that upon any infraction of this agreement by any of the minor children in
the vehicle, all monies paid, shall be forfeited and all limousine or bus service will be
immediately terminated.

LIMO4ME.COM reserves the right, to not allow any minor child to enter the vehicle
without a signed Parental Consent Form. Further more, no monies will be refunded to
any minor child who is barred from entering a vehicle without a signed Parental Consent
Form.
