    Dear Sir,

    This is your kind information , that free warranty period of one no. lift installed at your
    residence in Alambagh, is finishing on 20 february 2017. So we are submitting our AMC
    proposal for proper operation of lift for your acceptance. In accordance with the term
    and condition as per our offer for AMC of G+2, Automatic door lift as follows,

    Annual Maintenance Contract charges of one no. elevator of G+2,

    Rs. 30,000/-(Rs. Thirty thousand only)+service tax @15% .

    Payment terms-

    100% value of AMC at the time of agreement .

    NOTE--Payment shall be made in favor of RELIVO ELEVATORS PVT. LTD.

    In Elevator maintenance contract, we undertake two types of maintenance:

    1. Periodical maintenance
    2. Breakdown maintenance

    Periodical maintenance: Under this maintenance, our well trained & qualified
    technician will visit your site once in a month and carry out following activities:

    1. Thoroughly inspection of elevator/lift.


    2. Checking of oil level in gear machine.
    3. Necessary cleaning of elevator/ lift.
    4. Necessary lubrication of guide rail and rotating parts.
    5. Testing of all safety devices.
    6. Carry out minor necessary adjustment.

    Breakdown Maintenance: Breakdown maintenance means when elevator/lift is not


    working, in this case our service engineer will be attended the elevator/lift when your
    call is registered.

    Terms and conditions of Elevator maintenance contract:


    1. proposal when accepted & confirmed by our authorized official shall constitute the
    contract This between us & all prior representation & arrangement not
    incorporated herein are superseded.
    2. This contract does not cover any alternation, modernization or modification jobs.
    3. All the works shall be carried out during the normal working hour of our regular
    working days. Emergency service will be round the clock.
    4. Breakdown call will be attended on the same day.
    5. The customer shall provide proper lighting arrangement in Machine room, Hoist
    way, cabin top, pit etc.
    6. Any abnormality observed in the lift should be immediately reported to us.
    7. We reserve the right to cancel the contract if we find elevator is tempered by any
    unauthorized person.
    8. Full payment will have to be made in advance by cheque/DD in favor of Relivo
    Elevators pvt. Ltd. payable at lucknowi.
    9. This maintenance contract is valid for the period mentioned above subject to full
    and final payment of contract charges, as stated here in above is received in
    advance.
    10. This contract papers are valid only if the same is finalized on or before 30
    days.

    EQUIPMENT COVERED UNDER MAINTENANCE CONTRACT,


    Repair or replacement of printed circuit board and sub components inclusive of rollers,
    solinoids coils, brake shoe liners , contactors, V3f drives, contact relay, push buttons,
    indicators, magnetic switch, car and landing gate contacts, door shoes, car shoe
    liners, interlocks, and other minor parts to be covered.

    Annual Maintenance Contract Duration:

    One year From 21/02/2017 to 20/02/2018
