Directions: This self-evaluation form was developed based on the minimum expectations for all teaching
faculty. Any teaching faculty member seeking contract renewal or promotion should complete the following
self-evaluation providing an explanation for any performance criterion rated as "Above Expectations" or
"Needs Improvement".
Faculty members seeking contract renewal only:
should submit this form to the appropriate Academic Dean by the deadline listed in the Faculty/Staf
Manual
can opt to submit an electronic portfolio in addition to this form
at the rank of either Instructor or Assistant Professor should include "Above & Beyond" example(s) if
applicable
above the rank of Assistant Professor should include "Above & Beyond" example(s) corresponding to
the "Performance & Promotion Criteria for Professional Academic Rank" section of the Faculty/Staf
Manual
Faculty members seeking promotion should:
briefly highlight activities supporting your promotion in the relevant "Above & Beyond Examples"
sections below
refer the supervisor to the associated section(s) of the portfolio for more detail about these activities
submit this form to the Dean
consider submitting this form as part of your portfolio to support your application for promotion
The Academic Dean will then review the faculty member's self-evaluation in addition to student
evaluations, classroom observations, review of course materials, review of annual goals, and any other
relevant information to complete the Supervisor Evaluation.
