Digos City, and maintains a regular place of work at Jose Abad Santos
St., Digos City;
2. That she is a member in good standing of the Philippine Bar,
Integrated Bar of the Philippines and the Local IBP Chapter where she
belongs;
3. That she possesses all the qualifications and none of the
disqualifications for the Office of a Notary Public;
4. That she was never convicted of any crime involving moral turpitude;

6. That she is a person of good moral character as certified by two (2)

executive officers of the IBP, Local Chapter, copies of which are hereto
attached to form part of this petition;
7. That attached hereto are clearances from the Office of the Bar
Confidant of the Supreme Court and the Integrated bar of the Philippines
to form part of this petition;
8. That attached hereto is the receipt of the payment of the application
fee as proof of payment for the filing of this petition to form part of this
petition; and
9. That attached hereto, in a separate sheet of paper but a part of this
petition, are three (3) specimens of petitioners official signature, true
impression of her dry seal and three (3) passport size colored
photographs.

WHEREFORE, premises considered, it is most respectfully prayed of
this Honorable Court that petitioner be issued a commission as Notary
Public for and in the Province of Davao del Sur for the period starting
January 1, 2017 to December 31, 2018.
Petitioner prays for other reliefs just and equitable under the premises.
