The decedent was a 21-year-old single Black male who was active duty with the United States Navy and resided on base
at the Naval Air Station North Island. On the morning of 01/01/16, a driver traveling northbound on the ISR 5, just south
of J Street saw the decedent's severely damaged vehicle against a tree on the right shoulder of the highway. 911 was called
and police responded to the location along with fire personnel. Upon arrival, paramedics confirmed his death due to
obvious trauma to the body. The accident was unwitnessed and there was no evidence of the decedent braking.
Medical Examiners jurisdiction invoked according to the California Government Code 27491: Deaths due to known or
suspected as resulting in whole or in part from or related to accident or injury, either old or recent.

INVESTIGATIVE NARRATIVE
Decedent: Vian Stafford Irving II
Antemortem Events:
On 01/01/16, Officer Sibayan ID 17990 of the California Highway Patrol-San Diego Division provided the
following preliminary information during a personal interview at the scene. On the morning of 01/01/16, a
driver traveling northbound on ISR 5, just south of the J Street over crossing saw vehicle with severe damage on
the right shoulder against a tree. 911 was called and the dispatcher was advised of the accident.
The California Highway Patrol-San Diego Division Dispatch Center received the call at 0701 hours and
numerous officers were dispatched to the location along with the Chula Vista Fire Department. Officer Efrem
Moore ID 16542 was first on scene at 0715 hours and found the decedent in the driver seat, not breathing.
Paramedics arrived shortly after and confirmed his death due to obvious trauma to the body. The Medical
Examiner's Office was contacted at 0715 hours and the scene was secured pending my arrival.
Officer Sibayans preliminary investigation indicates that the decedent was traveling northbound on the ISR 5,
just South of J street overcrossing when he drifted onto the shoulder, struck a highway sign, overturned and
struck a large tree. Vehicle debris and personal effects were strewn in the area. There were no known witnesses
to the accident and the engine compartment was cold to the touch.
Past Medical, Surgical, and Social History:
According to military staff, the decedent had an unremarkable medical history and his social history was
unknown.
Scene Description:
On 01/01/16 at 0805 hours, I viewed the scene which was the right shoulder of the northbound ISR 5, just south
of the J Street over crossing. The Northbound ISR 5 has three lanes of travel and dedicated J Street/Marina
Parkway exit. There are trees that line the shoulder embankment with vegetation. A damaged highway sign was
on the ground with vehicle debris against one of the trees. The vehicle, a blue 1998 ZXS Ford sedan with VA
license plate VHH5822 was facing southeast against a large tree just north of the initial impact. The vehicle
sustained major overall damage and the front end of the vehicle had broken off and was north of the vehicle
down the steep slope. There was vehicle debris, clothing and personal effects strewn near the vehicle. The
decedent was located in the driver seat with major intrusion to the driver side door and roof of the vehicle. Fire
personal had to return to the scene to extricate the decedent from the vehicle. The decedents personal property
was collected and inventoried at the Medical Examiners Office.
Body Description:
Upon further review, the body of a Black male was viewed seated in the drivers seat of the vehicle, seat belted
with his left arm extended out the door along with his legs due to major intrusion of the cab. He was clad in a
jacket, camouflage Navy uniform, and pair of black boots. He was warm to the touch and flaccid. He sustained
severe blunt force trauma to the head, arms and lower extremities. Due to the heavy traffic in the area, the body
examination was limited.

92M Transport personnel, Partida and Arenas arrived and assisted fire personnel with the removal. The body
was placed into a new white vinyl pouch and a yellow identification band was placed on the right ankle. Blue
tamperproof seal 4141872 was affixed to the pouch at 0840 hours and the body was transported to the Medical
Examiner's Office for examination.

The decedent was visually identified using a photograph comparison from his military identification card.

Other Important Factors:
I contacted the United States Naval Casualty Assistance Call Officer (CACO) and advised them of the death.
They were able to contact his command and notified them of the death. I learned that the decedent was living on
base at the Naval Air Station North Island located at 281 Murray Road, San Diego, CA 92135. They would
have the family notified of the death and assist them with the funeral arrangements.

I.
Multiple injuries:
A.
No significant injuries of the head, neck or torso.
B.
Closed fracture of the proximal left femur.
C.
Open fracture of the distal left tibia and fibula.
D.
Fracture-dislocation of the symphysis pubis.
E.
Multiple abrasions and contusions of the neck and extremities.
II.
Other findings:
A.
Focal marked atherosclerotic changes of the proximal left anterior
descending coronary artery, estimated 75% stenosis; distal left anterior
descending and other major coronary arteries are unremarkable with no
significant atherosclerotic changes.
B.
No evidence of remote or acute myocardial infarction.
C.
Bilateral pulmonary edema.
III.
Toxicology:
A.
Alcohol (ethanol): 0.03% in blood and 0.04% in vitreous.
B.
Cannabinoids (marijuana): Delta 9-THC 5.0 ng/mL, and Delta 9-Carboxy
THC 9.0 ng/mL.
C.
No other screened illicit drugs of abuse detected.

EVIDENCE OF INJURY
BLUNT FORCE INJURY OF HEAD & NECK:
Examination of the head and neck revealed no significant injuries. Minor superficial
contusions and abrasions are noted on the left forehead and right chin. A laceration of the
right eyebrow is noted. Linear discoloration of the anterior neck is noted, possibly a skin
fold. Mild localized subgaleal hemorrhage is noted under the left forehead abrasion and
the right eyebrow laceration with no associated fractures. Examination of the cranial cavity
and soft tissues of the neck reveals no internal injuries.
BLUNT FORCE INJURY OF TORSO:
External examination of the torso reveals no significant injuries. A small superficial
contusion is noted on the left upper chest/clavicle, a superficial abrasion on the left lower
abdomen, and a small laceration of the right upper chest/shoulder are noted. Internal
examination reveals no internal injuries of the chest and abdominal cavities. Examination
of the pelvis reveals fracture-dislocation of the symphysis pubis, with soft tissue
hemorrhage on the intact urinary bladder and prostate.
BLUNT FORCE INJURY OF EXTREMITIES:
Examination of the upper extremities revealed lacerations of the dorsum of the right hand,
and minor superficial abrasions and superficial laceration of the right forearm. The left
upper extremity reveals no significant injuries. Examination of the lower extremities
reveals abrasions of the medial surface of the proximal right thigh, contusions-abrasions of
the right leg and foot, laceration of the right shin, and large laceration of the left shin.
There is deformity of the left thigh with closed fracture of the proximal femur, and open
fracture of the distal left tibia and fibula.
INTERNAL EXAMINATION
Injuries described above will not be repeated.
INITIAL INCISION: The body cavities are entered through the standard coronal incision
and the standard Y-shaped incision.
BODY CAVITIES: The abdominal fat layer measures up to 2 cm in thickness. The body
cavities have no hemorrhage or abnormal fluid. The serosal surfaces are smooth,
glistening, and without adhesions. The organs are normally located. The diaphragm is
intact. The body cavities have no internal injuries. A small blood clot is noted on the
anterior surface of the right lung. No obvious source of hemorrhage is noted.
CARDIOVASCULAR SYSTEM: The pericardium is thin and smooth. Within the
pericardial sac, there is no blood or excessive fluid. The heart weighs 350 grams and is
not enlarged. It has a normal shape with a smooth, glistening epicardium. The coronary

arteries have a normal origin and distribution with right dominance. The proximal part of
the left anterior descending coronary artery (LAD) reveals 50-75% atherosclerotic
stenosis. The rest of the LAD, and other main coronary arteries reveal no significant
atherosclerotic changes and are widely patent.
The chambers are normally developed and are without mural thrombosis. The
myocardium is red-brown, firm, and uniform without focal fibrosis, softening, or hyperemia.
The ventricles are not dilated or hypertrophied. The right ventricle, left ventricle, and
interventricular septum measure 0.3 cm, 1.3 cm, and 1.2 cm, respectively.
The endocardium is intact, smooth, and glistening. The cardiac valve leaflets are of
normal number, pliable, intact, and free of vegetations. The tricuspid, pulmonic, mitral and
aortic valves measure 13.0 cm, 7.0 cm, 11.0 cm, and 7.0 cm, respectively. The atrial and
ventricular septa are free of defects. There is no abnormality of the apices of the papillary
musculature.
The great vessels enter and leave in a normal fashion. The aorta follows its usual course
and has no atherosclerotic changes. There are no vascular anomalies or aneurysms. The
vena cavae and pulmonary arteries are without thrombus or embolus.
RESPIRATORY SYSTEM: The right and left lungs weigh 520 and 630 grams,
respectively, and have the usual lobation. The pleura are smooth and glistening; the lungs
have minimal to mild anthracotic pigment. The lungs are well expanded and crepitant.
The parenchyma is dark red and exudes moderate amounts of fluid. The lungs have no
consolidation, hemorrhage, infarct, tumor, gross fibrosis, or enlargement of airspaces. The
bronchi contain no foreign material and have unremarkable mucosa. The hilar lymph
nodes are not enlarged.
HEPATOBILIARY SYSTEM: The liver weighs 1230 grams, is of average size. The intact
capsule is smooth and glistening. The parenchyma is red-brown and uniform without
mass, hemorrhage, yellow discoloration, or palpable fibrosis. The gallbladder contains an
estimated 10 ml of bile and no stones. Its mucosa is uniform and the wall is not thickened.
The pancreas has a normal size, shape, and lobulated structure. The parenchyma is pinktan, firm, and uniform. The pancreatic ducts are not ectatic, and there is no parenchymal
calcification. There is an area of superficial hemorrhage on the anterior surface of the
pancreas with no associated injury. The rest of the pancreas and head are unremarkable
with no evidence of trauma.
HEMOLYMPHATIC SYSTEM: The spleen weighs 140 grams. The capsule is smooth
and intact. The parenchyma is maroon, firm, and uniform. There is no enlargement of the
lymph nodes in the neck, chest, or abdomen.

ENDOCRINE SYSTEM: The thyroid gland is not enlarged, and the lobes are symmetrical.
The parenchyma is uniform, firm, and red-brown. The parathyroid glands are not
identified. The adrenal glands have the usual size and shape. The cortices are thin,
uniform, and yellow, and there is no hemorrhage or tumor. The pituitary gland is not
enlarged and is unremarkable.
GASTROINTESTINAL SYSTEM: The esophagus and gastroesophageal junction are
unremarkable. The stomach is not distended. The stomach contains approximately 400
ml of tan, partially digested and undigested food without visible pills or pill residue. The
gastric and duodenal mucosae are intact and unremarkable. The small and large
intestines are unremarkable to inspection and palpation and sectioning. The appendix is
present and is unremarkable.
GENITOURINARY SYSTEM: The right and left kidneys weigh 120 and 140 grams,
respectively, and have a normal shape and position. The cortical surfaces are smooth.
The kidneys have the usual corticomedullary structure without tumors or cysts. The
pelves and ureters are not dilated or thickened. The bladder contains 30 ml of clear yellow
urine. The mucosa is intact, and the bladder wall is not hypertrophied.
The prostate gland is of average size and grossly unremarkable. The testes are without
hemorrhage or tumor.
NECK: The tongue, strap muscles, and other anterior neck soft tissues have no
hemorrhage. The hyoid bone and the cartilaginous structures of the larynx and trachea
are normally formed and without fracture. The airway is unobstructed, lined by smooth,
pink-tan mucosa, and contains no foreign material. The cervical vertebrae have no
displacement, hypermobility, or crepitus.
MUSCULOSKELETAL SYSTEM: The musculoskeletal system is well developed and free
non-traumatic deformity. There are no fractures of the clavicles, sternum, ribs, or
vertebrae. The ribs are not brittle. The skeletal muscle is dark red and firm.
CENTRAL NERVOUS SYSTEM: The scalp is intact. The calvarium and base of the skull
are normally configured and have no fractures. The dura is intact, and there is no
epidural, subdural or subarachnoid hemorrhage.
The unfixed brain weighs 1470 grams. The leptomeninges are glistening, thin and
transparent without underlying hemorrhage, exudate, or cortical contusions. The
hemispheres are symmetrical and have a normal gyral pattern. There is no flattening of
the gyri, narrowing of the sulci, midline shift, or evidence of herniation. The arteries at the
base of brain have no atherosclerotic changes or aneurysms.

Sections through the cerebral hemispheres have a uniform, intact cortical ribbon and
uniform white matter. Anatomic landmarks are preserved, symmetrical and without focal
change. The ventricles are not enlarged without dilation or distortion, and the linings are
smooth and glistening. The pons, medulla, and cerebellum are unremarkable, without
focal lesions. There is no evidence of uncal or cerebral herniation.
SPECIMENS RETAINED
TOXICOLOGY: Samples of central and peripheral blood, vitreous humor, urine, liver and
gastric contents are retained for toxicology. Toxicology testing were positive for alcohol
(ethanol) and cannabinoids (marijuana).
HISTOLOGY: Representative sections from the major organs are preserved in one
storage jar in 10% formalin. Representative sections of major organs submitted for
histology in 5 cassettes labelled:

MICROSCOPIC EXAMINATION
CORONARY ARTERY: Focal 75% atherosclerotic stenosis. The other sections reveal no
significant stenosis.
HEART, LUNGS, LIVER and KIDNEY: Passive congestion, postmortem artifact, and no
significant pathology.
