PRC Authorization Letter

Amor A. Betonio wrote an authorization letter allowing his co-teacher, Rosemarie P. Atup, to claim his PRC ID from the Professional Regulation Commission office in Davao City on his behalf. Dear Maam/Sir:
I, the undersigned name is sending you this letter as notification
that I am allowing /authorizing the bearer of this letter, Mrs.
Rosemarie P. Atup my co-teacher to claim my PRC ID in your
office.
I wont be able to claim it personally because of an important
reason that is why I have asked her to bring this letter together
with my identification card as valid proof. I am also giving her the
right to sign any papers that has connection with the claiming of
my PRC ID.
