Attached herein as ANNEX A-series are copies of the incident reports filed and submitted by the above-named witnesses and complainants. How did the offense adversely affect the company: The notoriously undesirable attitude of Ms. Antioquia affected the morale of the companys employees most particularly in her account (Citibank) which is the companys biggest client. An employee
demoralized by a much senior employee would definitely affect the
production of the employee as well as the entire team. To date, the production output of the Citibank team has decreased since the undesirable attitude of Ms. Antioquia affected her teammates. Furthermore, the complaints and incident reports submitted by her colleague suggests that Ms. Antioquia manifests ill will in her work and to the company management.
