    The patient presented with itchy, flaky skin due to seborrheic dermatitis. On examination, inflammation, erosions, and blisters were observed. The nursing diagnosis was impaired skin integrity. The plan was to encourage skin care routines, apply topical steroid creams, advise against scratching, and prepare the patient for phototherapy to improve skin integrity and decrease itching and flaking within 8 hours. Interventions included promoting healthy skin, using anti-inflammatory drugs, avoiding triggers, and considering lifestyle changes. Evaluation found improvement in skin and decreased itching and flaking after 8 hours.
