PETITION FOR ADOPTION

COMES NOW the petitioners, by the undersigned counsel, and unto

this Honorable Court, respectfully aver:
1. That the petitioners are husband and wife, both of legal age, and
residents of Paco, Manila;
2. That they have no legitimate children or descendants and hereby desire
to jointly adopt a child, named Julia Salvacion, who is seven years of age,
legitimate child of Spouses Julio and Gina Salvacion.
3. That the father of said minor is now dead, and only her named mother is
alive;
4. That said mother, Gina Salvacion, is not insane, nor intemperate, nor
has she abandoned such minor, and with full knowledge of petitioners'
intention hereby expressly gives her written consent to the adoption, as
evidenced by Annex "A" hereto attached and made an integral part of this
petition;
5. That the petitioners are qualified to adopt the said minor, and will be
able financially and morally, to bring up and educate said minor, properly
and adequately.
WHEREFORE, it is respectfully prayed that upon due notice and
hearing judgment be entered adjudging that the minor child be freed from
all legal obligations of obedience and maintenance with respect to her
natural parents and that she be declared to all legal intents and purposes,
the child of the herein petitioners, and that her surname be changed to that
of the petitioners.

PETITION
NOW COMES, Kimmy Dora, the undersigned counsel, to this Honorable
Court and respectfully represents:
That she is the mother of Luke Dora, who is presently in the custody of
Diego Calbo, the maternal grandfather of Luke, who forcibly abducted
Luke and until now actually restraints him of his liberty;
That despite demands, Diego refuses to turn over the custody of Luke, to
the petitioner;
WHEREFORE, it is respectfully prayed that an order be issued to
Diego to bring the minor Luke to this Honorable Court at the hour and date
to be set by this Honorable court, and thereafter that the custody of the
minor Luke be turned over to the petitioner.

EXTRAJUDICIAL SETTLEMENT OF ESTATE
KNOW ALL MEN BY THESE PRESENTS:
This AGREEMENT made and entered into this 7th day of September
2015 by and among:
CHUVA CORNES, of legal age, Filipino, single, and with residence and
postal address at Road 59 Bangkal St., Makati City;
SONIA VILLAVA, of legal age, Filipino, married, and with residence and
postal address at Road 55,Bangkal St., Makati City:
TESS KONYAT, of legal age, Filipino, single, and with residence and postal
address at Road 56,Bangkal St., Makati City;
WITNESSETH:
THAT we are the legitimate children and sole heirs of GERRY J.
CORNES, who died on June 25, 2013 in Makati City; copy of his death
certificate is hereto attached as Annex "A";
THAT said deceased died without any will or testament and without
any outstanding debts in favor of any person or entity;
THAT said deceased, at the time of his death, left a certain real
property, situated in Makati City and more particularly described as follows,
to wit:
