    Dear Mr. (Name):
    This is to inform you of our lawful demand for a full payment and reimbursement for associated
    expense from a loan extended with you dated (please state date of contract) in the amount of
    (Philippine Peso)
    It is our intention to be reimbursed and remunerated from our transaction with you. You have
    patently failed to deliver on the prescribed time and manner of undertaking the contract that we
    have settled upon, the details of which is:

    Payment of Five Thousand Philippine Peso (P 5,000.00) monthly until the amount is fully
    paid

    We have already made repeated oral demands to you for compromises and subsequently a
    payment of the said loan and all have fallen into deaf ear. This is now our formal and final
    demand in writing of our refund claims against you. We would be waiting for your
    response/immediate action within 15 days.
    We wish that this be immediately settled amicably and peaceably without the need of
    adjudication or litigation. But if need be, we are also currently studying all other options in the
    collections of our claim against you and are more than willing to explore such options.
