Post Training Effectiveness Form

This form is completed by a manager or supervisor to evaluate the effectiveness of training a participant recently attended. It asks the manager or supervisor to note any changes in the participant's attitude at work, productivity, and ability to make better use of time and resources since attending the course. The form also requests any feedback the participant provided about the course, as well as any other comments the manager/supervisor has about noticeable changes in the participant or suggestions for improving future trainings.
