3.RequestforInformation(RFI)
Thisformwilluseifthereissomethingthatisnotclearinthedrawingtobetterunderstandorclarifycertainarea
ofthework,socontractorwillsendanRFItotheEngineerorConsultantforclarification.SeeheretheRFIform
inPDFordownloadRFIFormwordfile.
4.SubcontractorsApproval(SA)
ThisformwillusetosubmittotheEngineerforapprovalofanysubcontractorthattakesubcontractsworkina
project.SeeheretheSAform.
5.ConfirmationofVerbalInstruction(CVI)
ThecontractorwillusethisformtoconfirmtotheEngineerorConsultantfortheverbalinstructionhestated
duringmeetingorphonecallsorsitejointinspection.SeeheretheCVIformordownloadCVIformexcelfile.
IftheworkproceedswithoutsubmittingCVIisperformedatthecontractorsrisk.
6.InspectionRequest(IR)
Thisformwilluseifthereispartofactivitythatisreadyforinspection.Theinspectionrequestshallbe
submitted24hoursbeforeinspection.IRshallnotbeacceptedbytheEngineerifthetimeofinspectionisless
than24hourssotakecareaboutthetimeitmightcreatedelaysonyourprojectifitisaccumulated.
Succeedingactivitycannotproceedwithouttheapprovalofthepreviousactivity.Unapprovedinspectionrequest
canbethemediumtoholdpaymentsforrequestedprogressbillingofthecontractor.Itisalwaysnecessaryto
getapprovalforyourinspectionstoavoidpaymentsdelayissue.SeeheretheIRforminPDFordownloadIR

7.DailyReport
Thisformwillusetolistdownalltheactivityonsiteondailybasisincludingthemachineries,plant,manpower,
weatherreport.DownloadDailyReport.
8.WeeklyReport
Thisformwillusetolistdowntheweeklyprogressonsiteincludingpercentagecompleteoftheactivities.
DownloadWeeklyReportformwordfile.
9.ConcretePouringRequest(CPR)
Thisformwillusetorequestpouringconcrete.ItshallbesenttotheEngineerorConsultantonedayor24hours
beforethetimeofpouring.
10.NonconformanceReport(NCR)
ThisformwillusefornonconformitynoticedbytheEngineerandshallbeissuedtocontractor.Nonconformity
willexistifthespecification,drawingandotherstandardsarenotfollowedduringtheexecutionoftheproject.
TheNCRissometimesusedtoholdthepaymentsofthecontractorsbilling,nopaymentsshallbemadeunless
NCRisrectifiedandapprovedbytheEngineer.SeeheretheNCRformordownloadNCRformwordfile.
Thesearealltheprojectformswehandledfromthestartanduntilcompletionoftheproject.
