SCHEMA MODE INVENTORY (Version 1.

1)
This is a very valuable inventory developed by Jeff Young, the developer or Schema Therapy, and some of his colleagues.

To start the questionnaire, click on the


"Questionnaire" tab at the bottom of your screen

Complete the personal details: 'Name', 'Date of Birth', 'Highest level of Education' completed, and 'Today's Date'.
Read the Instructions.
Read each question and place your answer (1, 2, 3, 4, 5, or 6) in the grey box on the right side. Make sure you answer all the questions
and that the answers are in the grey boxes.

When you have completed the questionnaire, click on the


"Scoring Sheet" tab at the bottom of the sheet.

To print the Scoring Sheet, specify that you only want to print
pages 1 to 7 (see white arrows).
Save the document to "SMI V1.1 Surname, First Name"
For example, SMI V1.1 Blogs, John"
Email your document to me at [email protected]

Listed below are statements that people might use to describe themselves. Please rate each
item based on how often you believe or feel each statement in general using the frequency table.
