Enacted by the Philippine Legislature in 1971 as the Davao Public Hospital situated
in downtown Davao with a 25-bed capacity. Renamed Davao General Hospital around 1946
with a 200-bed capacity. Rebaptized as the Davao Regional Medical and Training Center in
1957 under R.A. 1859 with a 350-bed capacity to be established in Bajada with an area of
12.8 hectares. New building in Bajada inaugurated in December 1964 but transfer was
completed in 1966. Officially designated as Medical Center for Mindanao and Sulu by the
Department of Health under A.O. 157 effective January 01, 1971. The 200-bed capacity
Davao Medical Hospital was attached in 1986 and became under full administrative and
fiscal control of Davao Medical Center (DMC) by 1991. Upgraded in 1992 as 400-bed
capacity hospital for a total of 600-bed capacity for both main and the Mental Hospital.

- BED CAPACITY: 37 to 40
- NURSES STATION SET- UP: 1 Nurses Station or Ward with 1 Charge Nurse incharge of all types of patients including 3 Ward Staff nurses, 1 ICU and 1 NICU
nurses, and 2 OPD/ER nurses
- CONDITIONS OF SERVICE: 8 hours a day, 3-5 days a week either in the
morning, afternoon or night shifts.
- WORK SHIFTS AND TIME: Day shift: 0800-1600, Afternoon shift: 1600-2400,
Night shift: 2400-0800.
- NURSE-PATIENT RATIO: 1 Ward Nurse that handles about 5 to 10 various
patients that comes from these areas such as the Medical-Surgical, Pediatric,
Obstetrics and Gynecology rooms, and Admissions/Emergency room.

DETAILED DUTIES AND RESPONSIBILITIES


A. AS A PRE-OPERATIVE NURSE

Provides and maintains adequate facilities, equipments, medicines


and supplies for good patient care.
Obtains necessary data regarding the patients health history.
Keeps complete and accurate information reports and records.
Refers patient to surgeon specialist and medical internist of the
center.
Assists the doctor in eye examination, ultrasound and laser
treatment.
Performs electrocardiogram examination and hemogluco test.
Prepares schedule of operation and informs surgeon and
anesthesiologist who will undertake the surgery.
Gives pre-operative and postoperative instructions and adequate
health teachings to patient and family.
Admits and prepares the patient for his/her surgery.
Obtains consent from the patient/guardian before his/her surgery.
Takes and records patients initial Vital Signs such as blood pressure,
pulse rate, respiratory rate, temperature and weight.
Gives safe medication and treatment, noting patients reaction and
records them appropriately.
Assists the doctor in postoperative examination of the patient.
Performs wound assessment and dressing.

B. AS A SCRUB NURSE

Maintains and observes strict aseptic technique inside the Operating


Room.
Makes sure that the instruments and supplies needed in the surgery
are already prepared ahead of time.
Performs surgical hand scrubbing, gowning and gloving before
preparing and arranging the instruments and supplies into the mayo
stand and back table.
Serves gowns and gloves to the surgeon and assistant.
Assists during survey as Scrub or Circulating Nurse.
Serves the draping towels and sheets to the surgeon as soon as the
operative site has been prepared aseptically.
Serves the sterile instruments/supplies to the surgeon according to
standard procedure.
Saves all specimens for diagnostic purposes.
Counts sponges, needles, sharps and instruments with circulating
nurse before surgery and when the surgeon begins closure.
Assists the surgeon in putting the dressing/eye patch to the operated
site.

Does the after care of instruments and machines used in the


operation.
Arranges and packs the instruments for sterilization.

C. AS A CIRCULATING NURSE

Maintains and observes break in aseptic technique inside the


Operating Room.
Prepares the machines and equipment to be used in the operation.
Attends to the needs of any member of the O.R. team.
Helps in the opening of instruments and supplies to be used in the
operation.
Promotes physical comfort and safety in lifting, positioning,
restraining and transporting patients.
Attaches all monitoring device to patient such as cardiac monitor,
pulse oximeter and automated BP.
Performs nursing activities such as catheterization, suctioning and
oxygen administration per doctors order.
Assists the anesthesiologist during induction of anesthesia.
Prepares the operative site before the surgery by observing strict
aseptic technique.
Completes the intraoperative record.
Counts sponges, needles, sharps and instruments with the scrub
nurse and reports it to the surgeon.

D. AS A RECOVERY ROOM NURSE

Identifies nursing needs of patients by listening attentively to


endorsements.
Promotes physical comfort and safety of the patient.
Assists in moving, maintains proper body alignment and utilizes
proper body mechanism.
Carries out doctors order in conformity with hospital policy, legal
limitations and scope of nursing practice.
Prepares and gives safe medication and treatments noting patients
reaction and records it accurately.
Performs nursing activities such as catheterization, oxygen
administration, suctioning, vital sign monitoring and assessing.

Assists in the Operating Room as instrument nurse or circulating
nurse in the Recovery Room, provides immediate post-operative care
to patients prior to discharge.

Keeps all equipments, instruments in their proper place and ready for
use.
Does proper endorsement in the care of the patients.
Makes sure that the Operating Room and Recovery Room are ready
for use everyday.
Does proper recording of cases and charting which is a must following
the standard of safe nursing practice. Completes the chart of patients
by filing up items related to the operation performed. Always secures
patients consent for operation.
Logs in all cases in the logbook provided for.
Prepares the PHIC forms by the nurses-in-charge.
Maintains safety of the patient in all phases of surgical procedure.
Responsible for the transport, set-up, dismantling, turning on and off
and pinch setting on any machine on use.
Keeps track of supplies for laser surgery.
Responsible for the ultrasound machine, slit lamp, microscope or any
other machine that maybe related to the use of the laser machine.
Responsible for the laser lenses.
Responsible for securing schedule of laser surgery of charity patients
from Ophtha Consultant.
Keeps an accurate record of patients who underwent laser surgery at
the clinic.
Prepares and sets-up the mobile van during outreach activities. Does

1. Handled a case of a 60 years old, female, was admitted in CDUH due to R sided
weakness of the body with a diagnosis of Cerebro Vascular Disease and Hypertension
(HPN). Received patient awake on bed, conscious and coherent, afebrile, not in ARD,
with IVF of Plain NSS 1L @ 20gtts/min infusing well @ L arm, with the following V/S:
BP = 140/80 mmHg; PR = 78 beats/min; RR = 20 times/min; T=36.5 degree Celsius.
The following nursing interventions were done:

with a diagnosis of Pneumonia and Diabetes Mellitus (DM). Received patient from ER
per wheelchair, conscious & coherent, slightly dyspneic, with continues O2 inhalation
@ 2L/min via nasal prong, with IVF plain NSS1 L @ 20 gtts/min, febrile; with CBS of
148 mg/dl, with the following V/S: BP = 130/80 mmHg; PR = 82 beats/min; RR = 24
times/min; T = 38.3 degree Celsius; skin warm to touch. The following interventions
were done:
a) positioned comfortably in bed (semi fowlers position)

b) checked IVF site and regulated its rate


c) Paracetamol 1 tab given P.O as ordered then Q 4 PRN for T = 38 degree Celsius
and above
d) nebulization with combivent 1 nebule done then every 6 hours
e) chest and back tapping done every after nebulization
f) tepid sponge bath done
g) encouraged to increase oral intake of fluids
h) CBS monitoring done ac meals & @ bedtime; recorded & referred accordingly
to ROD
i) vital signs taken and recorded every two hours
j) turned to sides every two hours with back rub
k) assisted in moving in & out of bed to commode
l) passive exercises done on both upper & lower extremities
m) sponge bath and mouth care done every morning
n) intake & output measured and recorded every shift

3. Handled a case of 40 years old, female, was admitted in CDUH due to R breast mass, R
Mastectomy was done. Received patient from RR per stretcher, conscious & coherent,
not ARD, on NPO ; with IVF of D5LR 1 L @30 gtts/min infusing well @ L arm; with
dry & intact post op dressing, with hemovac in placed with bloody drainage, with
FBC to urobag in placed with light yellow drainage with the following V/S: BP =
110/70 mmHg; PR = 78 beats/min; RR =18 times/min; T = 37 degree Celsius
