extracurricular activities, relationship with friends and family, career plans and how the Post Graduate Programme
from IIMV fits into your dreams and ambitions. Please make sure that your essay forms a coherent whole.

ENCLOSURES & INSTRUCTIONS FOR FILLING THE APPLICATION FORM


ENCLOSURES:
1. Copies of SSC, HSC marks sheets.
2. Copy of bachelors degree mark sheets (semester wise/consolidated marks sheet) along with the degree
completion certificate. A copy of conversion formula from CGPA/GPA to percentage, if any.
a. If you are in the final year of bachelors degree, submit a bonafide certificate with signature and seal
from Registrar/ principal / Head of the Department of your college.
3. Copy of professional degree mark sheets along with the professional degree completion certificate if
applicable.
4. For each work experience entered, enclose a copy of work experience proof (First and latest payslip/
service certificate).
5. Copy of caste / disability certificate (NC-OBC / SC / ST /DA) if applicable
(all of the above documents need to be self-attested)
INSTRUCTIONS:
1. Primary Information:
The NC-OBC certificate is valid for one year from the date of issue; ensure that you submit a valid
certificate (valid till the date of registration to the programme -- end of July 2016).
The DA certificate must be issued by a Government hospital signed by a panel of three doctors or by
a Chief medical officer (CMO).
2. Academic Record Graduation:
If you are in the final year of bachelors degree, fill the section till the latest Year/semester marks
available, and attach the duly certified bonafide certificate.
Use the University prescribed rules for conversion of CGPA/GPA into equivalent percentage. If there
is no formula available for conversion, the equivalence would be established by dividing the
candidate's CGPA by the maximum possible CGPA and multiplying the result with 100.
3. Work Experience:
Part time / project / internship / pre-graduation work experience are not considered as work
experience.
Ensure that the work experience period entered does not overlap with period of graduate studies.
Job progressions / multiple designations within a company should be entered as a single work
experience. The designation should be the latest designation held within a company for a single
tenure.
BEFORE MAILING, PLEASE READ THIS FORM ONCE AGAIN AND MAKE SURE THAT YOU HAVE COMPLETED ALL
THE SECTIONS AND HAVE MET ALL REQUIREMENTS. PLEASE NOTE THAT INCOMPLETE APPLICATIONS WILL
NOT BE ACCEPTED.
