PURPOSE

The purpose of this policy is to define the Permit to Work (PTW) system, which provides a formal safe
system of control over Perro Negro 3 (PN3) activities, which are outside normal operating practice or which
have been identified as being potentially hazardous.
This procedure applies to all works that need a Work Permit

DEFINITIONS
Work Permit

These are formal documents specifying work to be done and precautions to be


taken. Work can only start when safe procedures have been defined and put
into place. The permit provides a clear written record, signed by a responsible
person, that all foreseeable hazards have been considered and all necessary
actions have been taken. It must be in the possession of the person in charge
of the operation before work can commence.

Cold Work

Cold work shall include but not be limited to the following: scaffolding,
bunkering, cleaning, over board working, painting (unless needle gunning),
insulation work, pressure testing, maintenance operations which require the
removal of critical HSE systems from service e.g. fire & gas, public address, life
saving or fire fighting systems etc.

Hot Work

Work that may generate a source of ignition releasing a quantity of energy


sufficient to ignite an explosive atmosphere

Hazardous Area

Area in which an explosive gas / air mixture is likely to occur in normal


operations

RESPONSIBILITIES
DRILLING OPERATION MANAGER

The Drilling Operation Manager shall be responsible for the implementation of this procedure, within his
areas of responsibilities in the Project. He shall ensure that PN3 employees are aware of PN3 Work Permit
System and the system is fully implemented onboard.

4.2

BARGE CAPTAIN

The Barge Captain shall ensure that:

All works requiring Permits are identified;


The Permit and associated certificates contain a clear description of the work to be done, its location
on the vessel, starting date and time, plus its duration;
The safety precautions to be taken and any isolation required are clearly detailed on the Permit and the
associated certificates;
Precaution shall be taken according to project risk assessment procedure;
Permits are only issued to trained competent individuals;
All other works, which are curried out at the same time and would create a hazard, are suspended.
Limitations on time and scope of work are defined;
The permit specifies the necessary action to be taken if the work has to be suspended before
completion;
All personnel responsible for the supervision and performance of the work are identified;
The work site is examined when work is completed/ suspended, before it is re-started and when the
work is completed to ensure that it is left in safe condition;
Permit to Work induction is given as a part of the safety induction procedure where a general
introduction is given to all persons who are onboard the rig for the first time.

SECTION LEADER / TEAM LEADER

It is the responsibility of the Section Leaders / Team Leaders to ensure that a Work Permit is issued
whenever it is required.
The Section Leaders / Team Leaders shall ensure that all the requirements specified in the Work Permit are
fulfilled and to check that the job to be done is not interfering with other jobs already in progress.
It is also the responsibility of Section Leaders / Team Leaders to ensure that, at the end of any work,
persons involved into the task have left the working site in safe condition.

INDIVIDUAL RESPONSIBILITIES

All individuals including PN3 crewmembers, Client, clients service companys representatives and Saipem
subcontractors working onboard the rig shall:

Have a good general understanding of the Permit to Work system and procedures that are operated in
any location at which they may have to work.
Have a detailed working knowledge of the Permit to Work system and procedures as it applies to their
own job.
Ensure that they do not start work on any job requiring a permit until a Permit has been authorised and
issued.
Ensure that the conditions and precautions set out and specified in the permit issued for work, which
they will be involved with, are fully understood.
Ensure that all precautions and safety measures specified in the permits and instructions are strictly
adhered to.
If any circumstance or condition changes, seek advice from the responsible person (Section Leader)
immediately.
If in doubt, stop and ask for assistance.
Carry out all work in a professional manner.
On completion of the task, inform the Barge Captain (day shift) / Tour Pusher (night shift) who will
ensure that the work site and equipment are restored to a safe condition before having the permit signed
off.

PROCEDURE
GENERAL

The Work Permit System shall include, but is not limited to the following operations:

Any hot work involving welding, burning, heating, any other spark producing activity or potential ignition
source.
Work requiring entry in any tank or confined space.
Work where is a danger of falling into the sea or any working over water activity.
Working involving the use of none approved electrical equipment.
Work on electrical or mechanical equipment requiring energy isolation to ensure the safety of personnel.
Working on electrical equipment in hazardous areas to eliminate source of ignition.
Work carried out below sea level
Work that might affect of disables the rigs safety system (ie. fire fighting, gas detection, watertight
doors etc)
Work involving performing or acidizing operations, radioactive materials, explosives or other dangerous
substances
Work on/or near moving equipment where safety barriers and guards have to be bypassed.
Work involving heavy lifts with materials handling equipment (ie, - cranes, BOP hosts etc) that are near
maximum safe working loads (85%).
Flaring and flow testing wells.
Man riding operations, this will include the use of a man-riding air hoist, work baskets whether
suspended by a crane or air hoist.
Personnel transfer to or from MODU to supply vessel.
Maintenance of the draw-works, pumps, rotary table, top drive systems, engines, generators, water
treatment plant, deep water wells or tanks.
Pressure tests.
Cementing operations.
Jobs above and on the Well Head Platform.
Installation and testing of all contractor equipment.

Commissioning of any machinery.


Any maintenance on the accumulator unit.
Slipping and cutting drill line.

Note: this list is not definitive, if in doubt, use the Work Permit.

5.2

FILLING IN THE WORK PERMIT

The person actually doing the job (or Work Group Leader) must fill in the Work Permit form from box 1
through box 8 (except item 4 & 6). Item 7 Gas Testing shall only be completed by qualified persons and that
person shall complete item 7 fully for all hot works and confined space entry. He shall sign into the box 9 and
pass the permit over to his Section Leader or Team Leader.
This Section Leader / Team Leader shall check the information reported into the permit. Whenever deemed
necessary he may write, into the box 8, any additional precaution to be taken and / or any extra protective
equipment to be used. The Section Leader / Team Leader shall sign into the box 10.
If it is necessary to analyze the atmosphere where the job will take place, the Work Group Leader or a Safety
Officer shall make the initial gas test and record the results into the Work Permit and / or in the Entry
Certificate (in case of jobs performed into a confined space).
Note: Any Work Permit (Hot work/Cold Work) has a maximum duration of 12 hours and does not exceed
over shift changes.

5.3

HOT WORK

If a job involves hot work, the permit must be submitted to the Clients representative onboard who will
confirm that the well is in safe condition for hot works by signing into the box 11. All permits considered
as hot work shall be completed on a Permit to Work form denoted by a red surround and section 2.
For any job that involves hot work at least one fireguard, that cannot be the person who is doing the job,
shall be appointed.
The fireguard shall have at least one fire extinguisher and be aware of the location of the nearest means of
fire fighting equipment.
The fireguard shall remain for the whole duration of the hot work in the vicinity of the working place and shall
check continuously the surrounding area to ensure that the sparks produced during the hot work do not
ignite any combustible material or do not affect other equipment.
The Section where the job is to be performed must provide the fireguard. Prior to start any hot work, the
fireguard shall inform the Control room about the particular area where the work is going to be performed.
As soon as the job is completed, the fireguard shall inform the Control room, so that the Radio operator will
re-set the fire alarm sensors.

Safety Instructions for Fireguards

Whenever deemed necessary more than one fireguard may be appointed to watch a hot work.
The fireguard shall not take part in any other work while hot work is in progress.
If the fireguard has to leave the work area he shall be replaced or, if not possible, the hot work shall be
stopped.
The fireguard or the Welder shall not leave the working site until the affected surface of the equipment /
structure / material is cooled.

The fireguard shall follow the instructions given by the Welder.

Pre Hot Work Check by Fireguard


Before the hot work is started, the Fireguard shall:

Check that there is no flammable material in the working site and in the close vicinity (surroundings);
Check that fire blankets are available in place;
Check that a fire extinguisher is available in place;
Locate the nearest fire alarm / telephone;
Check nearest fire hose box and make sure that it can be used.

Checking of Conditions while Hot Work is in Progress


The fireguard shall ensure that he has sparks under control. If not, the work shall be stopped until all
necessary safety precautions are satisfied.
In case the welding is on a bulkhead or a wall, the fireguard should check the other side of the wall prior and
while welding is in progress.
The fireguard shall also ensure that the conditions on the work site are not changing while the hot work is in
progress. In case of events like spilling of oil, general alarm, ventilation stop and break off oxygen, acetylene
or propane hoses, the hot work shall be stopped until the conditions are back to normal.

In Case of Fire
In case of fire the fireguard shall:

Activate the nearest fire alarm and / or alert a workmate who shall report immediately to the Control
Room.
Assist if required the Welder to shut down the equipment.
Try to extinguish the fire.

Checking of Equipment when Hot Work is Stopped / Finished


When hot work is stopped due to alarms, coffee or meal breaks, the Fireguard and the Welder, shall ensure
that oxygen, acetylene or propane bottles are closed.
Electrical equipment, when leaving the work area, shall always be turned off.
The Fireguard and the Welder, when the work is finished or when leaving the work site at the end of the day
and work is to be continued the next day, shall ensure that the conditions in the area are back to normal and
the work site is cleaned and in safe condition.

5.4

WORK REQUIRING ISOLATION OF ELECTRICAL POWER

All electrical isolations shall prevent any hazard associated with electrical equipment or electrically powered
machinery.
The electrical isolation shall ensure that machinery / equipment cannot be re-energized accidentally, by
locking off / tagging out.

In order to avoid confusion in identifying the correct equipment / part of equipment that has to be isolated,
the supervisor of the person who opens the Work Permit shall fill in and sign in the box 6 of the Work Permit;
then, the person who opens the Work Permit shall go to the electrician to ask for electrical isolation.
Prior to start any work that requires isolation of power, the Electrician on duty, after having isolated the
power, shall fill in and sign the Electrical Isolation Certificate form.
The Electrical Isolation Certificate shall be also signed by the Chief Electrician or Chief engineer on duty.
Copy of the approved Work Permit and Electrical Isolation Certificate shall be distributed accordingly:
Original Certificate to the Control room attached to the respective Work Permit, 1 copy to the permit holder
and second copy to the relevant switch board.
.
Should machinery remain out of order for a period of time that exceeds 12 hours and isolation of electrical
power is necessary for the whole duration of the job then orderly transfer of Lockout/Tagout is required:

Both outgoing and incoming authorized employees assemble at lockout/tagout point


Outgoing employee removes his locks/tags
Incoming employee replaces with his locks/tags
Outgoing employee advises of any potential hazards
Appropriate isolation certificate will be filled and attached to the Work Permit

The padlock key of the lock out box must be attached to the original of the certificate and posted in the
control room.
Should the isolation of any emergency systems be required, only the part of the system relevant to the job
must be isolated.

Methods of Electrical Isolation

The secure isolation of equipment from all sources of electrical energy.


Discharge of any stored or induced electrical energy
The use of insulated mats, screens, gloves and tools.
The use of appropriately designed and rated test equipment.
Making sure persons doing the work are competent
The use of safe system of work.

To ensure the integrity of isolations once applied, the keys of padlocks used to effect the isolations will be
placed with a copy of the isolation certificate in a lock out box.
The Electrical Isolation Certificate form is in Red.
No alteration of the above colour is permitted (either by print or photo copy) as this may create confusion.

5.5

WORK REQUIRING MECHANICAL ISOLATION

Whenever maintenance or modification work is to be carried out on mechanical equipment, the Mechanic on
duty shall perform the isolation and a Mechanical Isolation Certificate shall be issued.
Mechanically driven equipment which has been shut down for repair or maintenance shall be disconnected
in such a way that unexpected re-start cannot occur.
The method of isolation shall be recorded in section 2 of the Mechanical Isolation Certificate. Recommended
methods of isolation are:

Physical disconnection
Spades or line blinds
Double block and bleed off valves
Closed and locked valves

All mechanical isolations shall have a Mechanical Isolation Certificate issued in association with a relevant
Work Permit.
In order to avoid confusion in identifying the correct equipment / part of equipment that has to be
disconnected, the supervisor of the person who opens the Work Permit shall fill in and sign in the box 6 of
the Work Permit; then shall request maintenance section for mechanical disconnection.
A Do Not Operate tag shall identify any isolation.
The Do Not Operate tag shall contain the signature of the Mechanic that performed the isolation, the date
and the Mechanical Isolation Certificate number.
When the mechanical isolation is no longer required, upon request from the person whom opened the Work
Permit, the mechanic shall remove the isolations stipulated in section 2 of the Mechanical Isolation
Certificate. All Do Not Operate tags shall be removed and the certificate signed in section 5.
Should machinery remain out of order for a period of time that exceeds 12 hours and Mechanical isolation is
necessary for a longer duration, then orderly transfer of Lockout/Tagout is required:

Both outgoing and incoming authorized employees assemble at lockout/tagout point


Outgoing employee removes his locks/tags
Incoming employee replaces with his locks/tags
Outgoing employee advises of any potential hazards
Appropriate isolation certificate will be filled and attached to the Work Permit

lock out box. The Mechanical Isolation Certificate form is in Green.

Note: The Lockout/Tagout box shall be located inside the Radio Room

5.6

SIGNING OF WORK PERMIT PRIOR TO WORK

The Work Permit shall be signed, into the box 12, for approval by the Client Representative and Barge
Captain.
The Barge Captain is entitled to sign for approval all Work Permits (except Hot Work Permit) issued for any
job to be done, during his shift, in areas that are under the responsibility of the Rig Superintendent.
The Tour Pusher on duty in the night shift is entitled to sign the work permits as Responsible Supervisor for
any job to be done in their area of competency except of Hot Work Permit.
When signing for approval, the Barge Captain shall ensure that no other jobs, which may interfere with the
one that he is going to approve, are in progress. He shall also check that all the necessary protective
equipment and safety precautions are identified into the work permit and that the persons to be informed
about the work are identified.
He shall also decide if it is relevant to attach a copy of the "Safety instructions for pressure testing" or
"Instructions for Fire-Guards to the worker's copy of the Work Permit.

When signing Work Permits, the Barge Captain and the team leader shall evaluate if the work could also
have influence on other sections' work. If so, they shall discuss the work with the relevant Section Leaders
before issuing the Work Permit.
(e.g.
If the Maintenance Section need to shut down any equipment which are in their section and these
equipment may influence the drilling operation, the maintenance Section Leader shall notify the Rig
Superintendent to make sure that the work will not create any problem for the drilling operations in progress.
This also applies if they shut down a back-up system, which are normally not used. Then the Rig Supt. shall
at least be notified so he can pass the information on to his crew).
Any modification regarding changing of the persons nominated to sign for approval the Work Permit as
mentioned in paragraph 5.6 shall be made only by the Barge Captain who shall issue a Standing Order
referring to the matter.
Depending on the circumstances and whenever deemed necessary the Barge Captain may require
endorsing any Work Permit that has been already approved by Clients representative.

5.7

DISTRIBUTION OF WORK PERMIT/ CERTIFICATE PRIOR TO WORK

When the Work Permit and the relevant attachments (if any) are completed, the person who performs the job
shall submit them to the Radio Operator in the Control Room.
Upon receiving the Work Permit and the attachments, the Radio Operator in the Control Room shall:

Check if the permit has been filled in properly and duly signed
When relevant, check if the required attachments are present
Assign a progressive number
Post the first page (white) of the Work Permit on the notice board in the wheelhouse according to the
type of job (hot work, pressure test, etc) the Work Permit is referring to
Give back the remaining copies of the Work Permit to the person who performs the job
Fill in the Work Permit Log

Once the number has been assigned, the person who performs the job shall complete the distribution of the
Work Permit as follows:

DOCUMENTATION OF WORK STILL IN PROGRESS AT THE END OF THE SHIFT

In case that at the end of a working shift the job is not completed, the Barge Captain (day shift)/ Tour Pusher
(night shift) shall ensure that work place is left in safe condition and close the permit by signing into the box
13 on the original posted in the Control Room (Original page, white). The person who was performing the
job (or Group Leader) before coming to Barge Captain for notification, he shall collect all the other copies of
the permit and hand over to the Radio Operator for disposal.
If an Electrical Isolation Certificate was required and issued for the respective Work Permit, the electrician on
duty must be advised in order to extend the Electrical Isolation Certificate.
If a Mechanical Isolation Certificate was required and issued for the respective Work Permit, the mechanic
on duty must be advised in order to extend the Mechanical Isolation Certificate.
A new Work Permit shall be opened when the job will be resumed.
The Radio Operator shall file the closed Work Permit in a dedicated binder and shall fill in Work Permit Log.

5.9

DOCUMENTATION OF COMPLETED WORK

When the work is completed and the area is properly cleaned and brought to safe working condition, the
Barge Captain shall ensure that work place is left in safe working condition and close the permit by signing
into the box 14 on the original posted in the Control Room (Original page, white). The person who
performed the job (or group Leader) before coming to the Barge Captain for notification, he shall collect all
the other copies of the permit and hand over to the Radio Operator for disposal.
If an Electrical Isolation Certificate was required and issued for the respective Work Permit, the electrician
who opened the Electrical Isolation Certificate must be advised in order to sign it off. The electrician will sign
off the certificate only after checking that the relevant Work Permit was actually signed off.
The Radio Operator shall file the closed Work Permit in a dedicated binder and fill in Work Permit Log.
All permits shall be recorded in a Permit to Work Log that shall be maintained at the control room. The log
shall contain the following details; the date the permit was raised, type of permit, person and requesting the
permit, brief description of the work to be carried out, area where work will be conducted, the time and date
the permit was closed out.

5.10 ISOLATION SUSPEDED DUE TO TEST ON EQUIPMENT / MACHINERY


Any time an isolation (either electrical or mechanical) must be removed for testing the relevant equipment /
machinery, the person who opened the Work Permit shall fill in the box 8a of the Isolation Certificate,
requiring the withdrawal of the Isolation Certificate for testing.
The person who asked to re-connect the electrical / mechanical power to the relevant equipment / machinery
shall hold the Isolation Certificate until the test is finished.

If the test fails, the person who opened the Work Permit and required the withdrawal of the Isolation
Certificate shall sign in the box 8b of the Isolation Certificate for revalidation / extension of the isolation due
to test failure.
The Work Permit shall remain open until the job is finished or the 12 hours extension has expired.

5.11 LONG TERM ISOLATION


In the event it is necessary to isolate a piece of equipment for an extended period of time due to equipment
damage or waiting on parts to complete maintenance, the following procedure will be followed:

Work Permit will be raised and an isolation certificate completed. A record of the isolation will be made in
the Isolation Log and added on the Active Isolation list displayed in the Radio Room. The equipment will
be isolated, locked and tagged out. When isolation of the equipment has been completed (i.e.-removal of
the equipment) all safeguards will then remain in place and work permits will be opened/closed during
further work/repairs process on the equipment.

When work/repairs have been completed on the isolated equipment, the following steps will be followed.
First a new Work Permit will be raised to cover the de-isolation process. All lockout/tagout equipment will
be removed following all safety procedures. Once the equipment is back in service and all system have
been returned to a safe condition, both Permits will be cancelled.

Long Term Isolation Log shall be maintained and kept up to date at Radio Room. A simple log sheet with
columns to record the following minimum details will be satisfactory: Original permit number, date of
isolation, type (electrical or mechanical), equipment or system isolated, reasons for isolation, date of deisolation and Cold Work Permit number to perform de-isolation.

5.12 SPOT CHECKS


The Work Permit system shall be monitored to check its effectiveness.
The Safety Officer shall carry out spot checks and sign into the appropriate box on the copy of the Work
Permit posted in the Radio Room.
The system will also be audited during regular HSE Management System Audits and will be reviewed and
revised as necessary to maintain its effectiveness.

5.13 JOB SAFETY ANALYSIS


The Job Safety Analysis shall be discussed and conveyed to all personnel involved in the task that Work
Permit is opened for according to the procedure PN3-PRO-HSE-032.
The copy of the JSA report is to be kept in a file with the permit to work and attached to the appropriate
permit.

All equipment to be used during the pressure testing shall be approved and certified for the pressure
requirements as per specifications;

All manometers shall be calibrated;

The responsible Team Leader shall ensure that system and equipment are blinded according to
procedure PRO-TEC-001 Installation / removing of blinds and opening of flanges"

The pressurized system shall be equipped with bleed off valve to remove possible trapped pressure;

A low-pressure test shall be performed and the pressure must be increased by steps of 25% increments
until test pressure is reached.

Area where high pressure testing is in progress shall be sealed off and warning signs to be placed.

Prior to start any pressure test operation, the area shall be barrier off properly. If it is the case, additional
warning notices are to be posted in the access points.

Before the high-pressure test starts, warning shall be given on the P.A. system onboard.

When the high-pressure test is finished all barriers and warning signs shall be removed.

Notice shall be given on the PA system when the testing is finished and the area is cleared.

All warning barriers, notices shall be removed immediately when the pressure test is finished.

ENTRY INTO CONFINED SPACE

An Entry Certificate is used to specify the precautions that need to be taken to eliminate dangerous fumes or
prevent a lack of oxygen before a person is permitted to enter a confined space. The Confined Space Entry
certificate shall confirm that the space is free from dangerous fumes or asphyxiating gases.
Precautions shall be specified on the certificate to protect the atmosphere against the ingress of airborne
contaminants from adjacent sources. A Work Permit for entry into confined spaces cannot be issued without
the Entry Certificate having been correctly completed; this certificate has to be used with relevant Work
Permit.
