Obsessive Compulsive Disorder

Obsessive Compulsive Disorder (OCD) is characterized by intrusive thoughts, rituals, preoccupations, and compulsions that interfere with daily functioning. Obsessions are recurrent, distressing thoughts and compulsions are repetitive behaviors performed to reduce distress. OCD is the fourth most common psychiatric diagnosis and is related to the anal-sadistic psychosexual stage of development. The four major symptom patterns are contamination, pathological doubt, intrusive thoughts, and need for symmetry. Treatment involves pharmacotherapy with SSRIs or clomipramine and behavior therapy such as desensitization, thought stopping, and flooding.

Notes on Obsessive Compulsive Disorder


Obsessive Compulsive Disorder - represented by diverse group of symptoms that
include :
1. Intrusive thoughts
2. Rituals
3. Preoccupations
4. Compulsions
- interferes with the person's normal routine, occupational functioning,
Usual social activities / relationships
Obsession - recurrent and intrusive thoughts, feelings, idea or sensation;
A mental event.
Compulsion - is a behavior, conscious, standardized, recurrent behavior
As counting, checking, avoiding
- ego dystonic
- irrational, distressing
- 4th most common psych dx after phobia, substance and MDD
- psychogenesis - related to anal sadistic phase
- essential feature of OCD : obsession & compulsion
Four major symptom pattern in OCD
1. Contamination - most common, seen in repetitive washing
2. Pathological doubt - 2nd most common, as in compulsive checking
3. Intrusive thoughts - 3rd most common, repetitive thouhts of sexual/ aggressive
4. Symmetry - 4th most common , need for symmetry or precision
Ex hair-pulling, nail biting
Diff. Diagnosis :
1. OCD associated with Basal Ganglia disease (Sydenham's chorea, huntington's
disease)
2. Tourette
3. Obsessive Compulsive PD - ego syntonic
Treatment :
1. pharmacotherapy - SSRI FDA approved for OCD;
Clomipramine - 1st drug approved by FDA , most selective for
serotonin reuptake
2. Behavior therapy - beneficial effects are longlasting; approaches used are :
desensitization, thought stopping, flooding , implosion therapy,
Aversive conditioning
3. Psychotherapy - family therapy, insight oriented
4. Combination of both
Other disorders are :
1. Obsessice Compulsive or Related Disorder Due to Another Medical
Condition ex PANDAs
2. Substance Induced Obsessive Compulsive or Related disorder
Olfactory Reference Syndrome - characterized by false belief of px
That he or she has a foul odor not perceived by others.

BODY DYSMORPHIC D/o :


Characterized by preoccupation with an imagined defect in
appearance that causes significant distress or impairment in
important areas of functioning
HOARDING D/o :
Characterized by 1) acquiring of and failure to discard a large amt
of possession deemed useless or of little value;
2) greatly cluttered living area precluding normal activities;
3) significant distress and impairment in functioning due to
hoarding
Clinical features are :
- fear of losing items that px believes will be needed later and
A distorted belief about or an emotional attachment to
possesssion
- most hoarders do not perceive their behavior as a prob
HAIR -PULLING :
(Trichotillomania) a chronic disorderos
- repetitive hair pulling lead to variable hair loss visible to others.
There is increased tension prior to hair pulling or relief of tension
or gratification after hair pull.
Skin pricking :
- this is compulsive and repetitive pricking of the skin, associated
with repressed rage at authoritarian parents ; accd to
psychoanalist scratching skin or pricking is masturbatory
equivalent.
- most common site of skin pricking is Face
