Before completing the form, please note the following:
Please use block capitals on the application form;
You need to make a separate application for each candidate;
You need to make a separate application for each award.
There is a fee for this service, currently 75.00 per application. A cheque will only be accepted
if your cheque guarantee card details are entered on the reverse of the cheque. Payment
should be made to City & Guilds at the time of application. Payment may also be made by
credit or debit card, by completing the form provided.
Please read carefully the notes accompanying the application form.
Please allow a minimum of three weeks for the processing of the application. Please send

Notes on the Full Technological Diploma


To be eligible for the Full Technological Diploma the candidate must meet the following
criteria:
Candidate must supply copies of and have achieved a Technicians Diploma (applied) and
Advanced Technician Diploma (applied) in one of the following awards (except 2720)

These diplomas must have been awarded by City & Guilds.


Candidate must supply a portfolio of evidence which must include
1. A summary of the candidates work experience at least two years work experience at
a level appropriate to the advanced diploma syllabus
2. Letters from each employer confirming the candidates work experience at
supervisory level
3. A Curriculum Vitae (CV)
The portfolio may include any other information which the candidate considers relevant.
Candidates are requested to take the utmost care to ensure that the details entered on this
form are accurate to avoid delays in issuing the FTD.
