The candidates who do not complete 17 years as on 31-12-2014 need not attend the counseling. o Qualifying Exam: The candidate should have passed Intermediate (10 + 2 pattern) or its equivalent examination with Physics, Chemistry, Botany, Zoology and English. o The candidate should have qualified in EAMCET-2014. 10+2 (Sci %) 50% 45% 40% Compulsory Rural Medical / Dental Service: The Medical / Dental graduates admitted in the academic year 2014-15 in the State shall serve one year Compulsory Rural Medical / Dental service. Certificates to be produced at the Counseling Center o Original Hall Ticket of EAMCET - 2014. o Rank card EAMCET - 2014 in Original or Internet copy o S.S.C or equivalent examination showing the Date of Birth o Memorandum of marks in Intermediate or Equivalent Examination. o Transfer certificate of 10+2. o Study certificates from 6th Class to Intermediate. o Candidates who have studied in the institutions outside of Andhra Pradesh have to submit 10 years residence of the candidate/ parent issued by MRO / Tahsildar. o Caste Certificate (Integrated Community Certificate) who belongs to BC/SC/ST Category candidates issued by an Officer prescribed. Income and Service records & SSC / SSLC certificate of parent/ relative if parent is not in-service. o The candidates claiming fee reimbursement shall produce Income Certificate of Parent issued by M.R.O / Tahsildar on after 01.01.2014. o Fee reimbursement is allowed for SC/ST candidates for Govt, A and B category seats if the parental income is less than Rs.2,00,000/- Per Annum. Fee reimbursement is allowed for EBC/BC/PH candidates for Govt, and A category seats if the parental income is less than Rs.1,00,000/- Per Annum. o The Fee Reimbursement is subjected to the orders to be issued by the Govt. of Andhra Pradesh and Govt. of Telangana. The candidates shall submit an undertaking to the Principal stating that He/She will pay the tuition fee to the College by him/her self. COUNSELING PROCESS Reception: o Candidate will sign in the attendance register and note his/her Mobile Phone Number o The candidate will pay Non-Refundable counseling fee of Rs.500/- for OC/BC and Rs.300/- for SC/ST. Registration: Candidate will produce rank card and wait for registration. The staff will issue registration cum verification form Verification of Certificates: The BC/SC/ST Candidates will go to the respective welfare officer and get their castes certificates verified. The OC Candidates and Category Candidates after Caste Verification will go to the General Verification and submit their Certificates. Deposition of Certificates: The Verification Officer will collect Original certificates Pre Counseling: The Candidates will wait in the Pre Counseling area in the sequence of rank for their turn of Counseling Option Form: The Candidate will fill his/her data and sign in the bottom of the form with their parent/guardian signature and enter the date of counseling. Please do not fill your option at this stage. Counseling: On his/her turn, the candidate will be called at the counseling terminal for excersing his/her option of seat in a college with the help of counseling officer and note the same in the option form. If candidate does not like to opt a seat, he/she will note NO OPTION in the option form and hand over the staff. He/ She then leaves to the allotment area and wait. Fee Payment: The staff in the fee payment area will call the candidates to pay their University fee and the tuition fee Allotment: The candidate will be called to sign in the allotment register and allotment letter and collect the allotment letter. He / She will be also receiving receipt of certificates and receipt of fee paid. Reporting at the college: On or before 08-09-2014. Last Date of withdrawl: 08-09-2014 to be submitted at the Dr. NTR University of Health Sciences, Vijayawada along with cancellation fee Rs.1,00,000/ Classes: Commence from 01-09-2014.
Cancellation fee: Rs.1,00,000/- towards cancellation fee to be paid for cancellation of admission by the candidates who are provisionally selected into MBBS/BDS courses in the counselings for the academic year 2014-15 and also forfeit the University fee and tuition fee which was paid at the time of counseling
