I hereby solemnly affirm and declare:
(a) THAT the information given in this Admission Form is correct to the best of my knowledge and belief and
if anything is found incorrect, Quaid-i-Azam University (QAU), shall have the right to cancel my
admission and expel me from the University.
(b) THAT I promise to:
(i) abide by the Statutes, Rules and Regulations etc. framed by the QAU and any other instructions
issued from time to time;
(ii) maintain good behavior during studies and while being on Campus;
(iii) work diligently and maintain dignity and prestige of QAU both on and off the Campus.
(c) THAT I undertake to be a full-time regular student of the QAU and shall not join any other programme of
study or accept any employment for the duration of the programmes registered. (in-service candidates are
required to produce NOC/Study Leave duly sanctioned by the employers/authority).
(d) THAT I further undertake that I shall not claim hostel accommodation as a matter of right if admitted to the
QAU.
(e) THAT I promise to pay all the QAU dues regularly as and when required by the QAU.
(f) THAT I hereby declare as a binding on me that as long as I am a student I shall abide by all the Statutes,
Rules and Regulations, in force at the time of joining the QAU and framed subsequently. I shall submit
myself to the discipline of the QAU authorities as exercised through its various Committees and Officers.
(g) THAT the information given in this Admission Form is correct to the best of my knowledge and belief and
if anything is found incorrect, the QAU shall have the right to cancel my admission and expel me from the
QAU.
(h) THAT I shall not claim/apply for any financial support/fee waivers/exemptions once I am admitted to the
QAU on Self-Finance basis.

Originals of the above documents must be produced at the time of interview for admission.
Finally selected candidates for admission shall be required to produce a migration certificate (in original) on joining the
QAU.
Finally selected candidates for admission shall be required to submit an affidavit on a non-judicial stamp paper to the effect
that the certificates submitted by him/her are genuine and in case found fake/bogus/tampered the QAU shall have a right to
cancel the admission and initiate legal action against him/her. The candidate shall also be required to produce an affidavit in
accordance with the decision of the Supreme Court.
Candidates having obtained certificates/degrees from foreign universities/institutions should provide equivalence certificates
and conversion of grades/CGPA into marks or percentage of bachelors degree from Higher Education Commission (HEC),
Islamabad and for Secondary School (SSC) and Higher Secondary School (HSSC) certificates from Inter Board Committee
of Chairmen (IBCC). In the absence of equivalence certificates, foreign degrees/certificates shall not be accepted by the
QAU. Certificates/Degrees obtained from local universities/ institutions with CGPA also require conversion of grades into
marks or percentage from the university/institution concerned.
Candidates, who are more than 26 years of age, are not eligible for admission into Masters programme unless relaxation
(except in Management Sciences) is granted by the Vice-Chancellor for special reasons on the recommendations of
Chairperson of Department/School/Institute/Centre concerned.
In case of incorrect/misleading information or concealment of facts, the QAU shall have a right to refuse/cancel the
admission and expel the student from the University.
All entries in the form must be made in BLOCK LETTERS and/or typed.
In case, an entry does not apply to you, clearly write NOT APPLICABLE or N.A. In all other columns write the necessary
information or write appropriate replies, such as yes, no, nil, etc.
Incomplete application forms shall not be entertained (at any stage).
