    Brief description of the Near Miss:
    At Location No.85 tower erection was going on. One fitter was tightening nut and bolt
    at one leg at height of 10 m. One bolt falls down on ground. At that time nobody was
    under the tower. Two bolts and nuts were in his hand. When he was trying to insert
    one bolt inside the hole at the same time another bolt fall down from his other hand.

    Electrode with holder left at work site and welding machine was running.

    Date: 25.05.2010 Time: 10:50 A.M
    Brief description of the Near Miss:
    On 25th May11, Some of M/s. ITD labor are working in excavated pit depth 2.5mt.
    (Shuttering Work). One the labor is using a narrow excavated wall as access (both side
    excavated pits are there). At the same time he lose his balance and tripped on the same wall
    not in the excavated pit. If he felt either both sides it will lead to a major injury.

    Brief description of the Near Miss:
    On 09th May11, while shifting stones near to wall manually, one of M/s. A. Kumars labor
    got a minor injury due to stone which was dropped from labors hand. At the same time the
    labor wear safety shoe, so there is no major injury took place.
    Immediately TPL safety officer rushed to the location on M/s. A. Kumars supervisors
    phone call. Given First Aid to the victim and suggest to take rest for some time.

    Brief description of the Near Miss:

    On dt. 11/04/11 one of M/s. ITDs mason at retaining wall was jumped to down to escape a
    major incident. While doing mason work on wall (One side its height is 5mt. and 1mt. from
    one side), suddenly he lost his balance and jumped to lower height side, if not it will be a
    major accident at site.

    Date: 20.04.2010 Time: 03:15 P.M
    Brief description of the Near Miss:
    On 20th April11, One of M/s. ITD labor while entering into site tripped due to electrical
    cable laid on ground. While this near miss took place sharp edged stones and re-in forced
    bars (in the hands of labor) at the same location. Fortunately nothing was happed if not it will
    be major incident.
