Every time I complete a new
task, work with a new patient,
learn something new, work
through an issue with a client,
etc.- I will be mindful of any
feelings that come up and
identify them; good or bad.



I have been being mindful and
aware when I am feeling
nervous or overwhelmed while
at clinical, I have been
journaling about it and taking
some time after each clinical
day to reflect on how the day
went and why I felt this way-
ensuring I reflect and recharge
for the next day.


Have been expressing my
feelings to classmates when
Im nervous or unsure of
something, and utilizing our
post conference time to
reflect on how my day went.

Have been identifying all
feelings while working with
patients; i.e. I felt good about
how a conversation went or I
felt overwhelmed with what the
patient was telling me.

and recognize and reflect on
them when needed. I have been
making a point to monitor my
feelings while at clinical, and
either talk about them or reflect
in my journal.

critical thinking focusing on a
systemic method, problem
solving, and time management.



Have being assessing and
determining which client needs
take priority over the other and
managing my time to reflect

Have been communicating with
my patients to ensure which
needs are apparent and letting
them know when I can
complete certain tasks.

and taking notes on time

Will continue to utilize my
Drug guide and make sure I
know what the drug Im giving
is, what is the action, and what
is the indication for my patient-
therefor I can look for the right
effects

I will be aware of which side
effects to look out for, for each
medication; and continue to
monitor/assess my patient

Will be more aware to look for
changes/effects in my patients
before and after medications,
and continue to look through
the charts and identify changes
in behavior, mood, etc., since
start or change of medication

Will read my pharmacology
and the nursing process
textbook, for furthering
identifying the therapeutic
effects and side effects of
medications.



Have continued to utilize my
drug guide, making sure to
learn of the intended action,
and monitor my clients for
effects and side effects- for
instance the intended action of
the PRN- Ativan and how this
affects my patient.


Have been spending more time
looking through my patients
charts and their history,
determining what changes have
been occurring with certain
medications since starting
them.

Have been utilizing my
pharmacology textbook to
ensure I get the full scoop of
the meds Im giving and the
effects/side effects to look for.

confident in this area, I have
been recognizing the effects
and side effects of drugs and
making a point to monitor these
changes in my patients. I have
been noting the changes in
behavior over time with the use
of certain meds and making a
point to monitor changes after
med increases, decrease etc.

There is always room for
improvement, as there are
many more meds for me to
learn and more time I can
spend learning the effects and
side effects that present.

Stenberg resources on the acute
psychiatric nursing clinical
course- of specific skills and
competencies- to ensure my
nursing performance is
reflecting this.

journal how I am doing in
certain areas of performance-
positive or negative; and thus
make goals from there

Will Read chpt 11 in Principles
& Practice of psychiatry
nursing, focusing on
professional nursing
performance, p.159-169
Have continued to be self-

my progress- reviewing the
competencies and journaling
about tasks, skills I am
acquiring.

interacting my patients to

is reflecting my competencies
and theories learned in courses.

unit and my instructor if I am
unsure who to report my
observations to

Will continue to verbalize my
patients status, behaviors,
mood, etc. at 1:00 report while
on the unit

Will communicate with the
nurse on my team if they would
like me to inform them of any
issues/changes with that
particular patient for the day

Will review protocols for the
unit; to determine if a particular
incident comes up, who It
should be reported to





I have been reporting
appropriate information to my
nurse after we finish shift, and
continue to report appropriate
information at the 1:00
meeting.

I have checked with my
instructor and fellow nurses if I
should speak with the doctor
about a certain incident and
proceeding to do so.


Have reviewed the procedures
for reporting certain incidents
on the unit- for instance AWOL
procedures.

4/5 again for the most part I
feel confident in reporting my
observations to the appropriate
persons. There may be times
where I wasnt completely sure
but clarified with staff. Feel
confident I will seek out the
appropriate persons and ask if I
am not 100%.
Can continue to build on this in
different clinical settings.
