Note: this is not to be construed as legal advice. The above Proof of service is a very simple to do fill in
form that gives high credibility to any important documents you mail off. It provides for a witness that
can testify by swearing to the events. Coupled with a Certified Mail # or Registered Mail # you have
airtight evidence that the opposition 1. got the documents and 2. what time and date they received them.
Use regular postage ($.44) for inconsequential documents, Certified for very important documents and
Registered Mail (whose jurisdiction is now Internation and unquestionalble) for very, very important
documents.
You could move the lines up and paste a Jurat below to include a Notary stamp and make it into a
Notary Presentment. The Notary then acts an agent of the Court system swearing they put the
documents into an envelope and mailed them off. If they use their Notary return address for response
you have extremely good evidence that the Bond, Promissory Note, A4V etc was sent, received and
returned or not returned.

Instructions: fill in the court case #, loan # etc. at the top. Name the Documents so anyone reading them
will know they are the ones you sent. Like :1.”revocation of Power of Attorney” dated July 4th, 2009, 2.
“Affidavit of John Doe” dated July 9th, 2009, etc.
Your Domicile is where you the mailer live, it’s your address- 2000 Anywhere Street, (city) ‘Tucson’
The addressee is the entity you are mailing them to. i.e. IRS the mailed from is your address or city
Signature is by the swearing the above, the person who is ‘not a party to the case’ like your neighbor,
relative, friend, etc.
