graduate school policy that requires each school to submit an annual report documenting the progress of its graduate students and to facilitate collection of student data for TRACDAT. This APR, in conjunction with e aluation forms the faculty fill out each semester for their graduate TA!s, ad isees, etc., will be used to generate this report and collect student data. Please follow the format below when preparing your report and include all sections in the order listed. All bold headings should appear as headings in your report, but do not copy the non"bold instructions. #f there is nothing to report in a particular section, include the heading with an indication of nothing to report $i.e., %R or %&A'. Everything should be listed in chronological order from new to old. Please sa e an electronic copy of this report so that in subsequent years you will only need to update the report, not redo it from scratch. #t is ery important that you include dates with each of your accomplishments listed below. A graduate student evaluation form must be completed by your ad isor, all members of your graduate committee and by the professor in charge of the course$s' for which you were a TA. #t is your responsibility to pro ide the forms to the persons mentioned abo e. A copy of the form is at the end of this file $or printout'. Pro ide the faculty e aluations form to your committee members and to faculty TA mentors. +aculty e aluations should be submitted directly to the graduate committee by the faculty member by the l#st Fri"#y in Se$tem%er ,. -our ad isor will sign the APR and use it to write a summary of your progress for the year, documenting progress towards degree, strengths and wea.nesses. The ad isor will also e aluate whether you ha e addressed pre iously reported wea.nesses and then/ a' recommend continuation of the student0 b' recommend continuation with conditions0 or c' not recommend continuation. The ad isor will submit the APR and his&her summary to the chair of the 1chool of 2iological 1ciences 3raduate Committee by the l#st Fri"#y in Se$tem%er . When your APR has been approved by your advisor, send an electronic copy to the chair of the graduate committee. The electronic version will only be used to collected data such as publications, presentations etc for the evaluation committee . 4. The 1chool of 2iological 1ciences 3raduate Committee will e aluate the report, agree or disagree with recommendations from the ad isor, and submit the report to the school director by the L#st Fri"#y in O!to%er. +or doctoral students that ha e not yet passed their oral comps, the school director will agree or disagree and forward the report to the dean of the graduate school and the dean of the college. 5.
